冠心病患者外周单核细胞TLR4表达及其与血浆P选择素的相关性

【摘要】 目的探讨冠心病(CHD)患者外周血单核细胞Toll样受体4(TLR4)表达及其与血浆P选择素(P-selectin)水平的相关性和临床意义。方法选择研究对象139例,其中稳定性心绞痛(SAP)39例、不稳定性心绞痛(UAP)66例、急性心肌梗死(AMI)34例、正常对照组30例,比较各组间TLR4、P-selectin表达的差异。采用流式细胞术检测外周血单核细胞TLR4表达,采用酶联免疫吸附试验检测血浆P选择素水平。结果对照组、SAP组、UAP组与AMI组外周血单核细胞TLR4的表达和血浆P选择素水平依次增高;各组表达量均高于对照组(P<0.05),TLR4表达与血浆P选择素各组间两两比较差异均有统计学意义(P<0.05),TLR4与血浆P选择素水平呈正相关(P<0.05)。结论冠心病患者TLR4表达上调,促进炎症前体细胞因子的分泌,提示TLR4及其介导的炎性反应与冠心病的发生、发展有关;冠心病患者P-selectin水平升高,客观反映病情的严重性,预示心血管事件的危险性增加。

【Abstract】 Objective To investigate the expression of toll-like receptor 4 (TLR4) in peripheral blood monocytes in patients with coronary heart disease,and to explore the relationship between TLR4 expression and the level of P-selectin. Methods The expressions of TLR4 in peripheral blood monocytes from 30 controls,39 patients with stable anginapectoris (SAP),66 patients with unstable angina pectoris (UAP),and 34 patients with acute myocardial infarction(AMI) were detected with flow cytometry. The concentration of P-selectin in serum was measured by enzyme-linked immunosorbent assay (ELISA).Results Compared with control group,the expressions of TLR4 and the levels of P-selectin were significantly increased in SAP,UAP and AMI groups (P<0.05). There was a positive correlation between the expression level of TLR4 and the levels of P-selectin in the patients. Conclusion Coronary heart disease may be associated with over expression of TLR4 in peripheral blood monocytes. The high level of P-selectin in patients with coronary heart disease suggests the severity and increased risk of the cardiovascula disease.
