【摘要】 目的:探讨糖尿病、高血压、原发性慢性肾小球肾炎3种不同原发病所致早中期慢性肾衰的中医证候特点,揭示不同原发病所致早中期慢性肾衰在中医证候上的差异性与统一性。方法:采用横断面流行病学调查方式,对符合纳入标准的98例慢性肾衰患者进行临床流行病学问卷调查,归纳、分析其症状及中医证候,并对3不同原发病所致早中期慢性肾衰的证候特点进行总结、比较。结果:糖尿病肾病本虚证早期以脾肾气阴两虚以及脾肾阳虚为主,中期脾肾阳虚突出;在标实证中,早期以湿热、血瘀为主,中期以湿浊、湿热为主。高血压肾病早期以肝肾阴虚、脾肾气虚为主,中期脾肾阳虚、脾肾气虚为主;标实证以湿热与瘀血互为消长变化。肾炎后肾衰早期以脾肾阳虚为主,中期阳虚更甚,标实证以血瘀、湿热为主。结论:糖尿病、高血压、原发性慢性肾小球肾炎3种不同原发病所致早中期慢性肾衰的中医证候特点存在一定差异性。更多还原

【Abstract】 Objective:To study syndrome characteristics of chronic renal failure( CRF) at early and middle stage caused by di-abetes,hypertension and primary chronic glomerulonephritis,and reveal the differences and unity on syndrome characteristics of CRF at early and middle stage caused by different primary diseases. Methods:To use cross sectional epidemiological investigation,98 CRF patients accorded with the inclusive criteria were investigated. We make summary and analysis on the symptoms and syndromes,and summarize and compare the syndrome characteristics of CRF at early and middle stage caused by different primary diseases. Results:In primary asthenia syndrome of diabetic nephropathy,kidney-spleen qi-yin deficiency and kidney-spleen yang deficiency are predominant at early stage,and kidney-spleen yang deficiency is predominant at middle stage. In secondary sthenia syndrome,damp-heat and blood stasis are predominant at early stage,and damp turbidity and damp-heat are predominant at middle stage. In primary asthenia syndrome of hypertensive nephropathy,liver-kidney yin deficiency and kidney-spleen qi deficiency are predominant at early stage,and kidney-spleen yang deficiency and kidney-spleen qi deficiency are predominant at middle stage. In secondary sthenia syndrome,damp-heat and blood stasis are predominant. In primary asthenia syndrome of CRF caused by chronic glomerulonephritis,kidney-spleen yang deficiency is predominant at early stage,and yang deficiency is more seriousness. In secondary sthenia syndrome,blood stasis and damp-heat are predominant. Conclusion:There exist differential syndrome characteristics of CRF at early and middle stage caused by diabetes,hypertension and primary chronic glomerulonephritis.更多还原
