【摘要】 目的观察体外高频热疗联合中医辨证治疗对中晚期肝细胞癌患者在稳定病灶、延长生存期、改善症状、提高生活质量以及改善肝功能等方面的影响。方法收集我院2008年9月30日至2009年9月30日符合纳入标准的中晚期(ⅢB、Ⅳ期)肝细胞癌患者70例。采用随机数字表法分为治疗组(体外高频热疗+中医辨证治疗)37例和对照组(中医辨证治疗)33例。治疗6周后评价客观疗效、总生存时间(OS)、疾病进展时间(TTP)和6个月生存率。结果治疗组获稳定15例(40.54%),病情进展22例(59.5%);对照组获稳定9例(27.3%),病情进展24例(73.7%),两组差异有统计学意义(P<0.05)。治疗组的中位OS为5.2个月,中位TTP为2.3个月,6个月生存率为18.4%;对照组分别为3.7个月、1.6个月和12.6%,两组差异均有统计学意义(P<0.05)。治疗后两组生活功能指标量表(FLIC)评分差异有统计学意义(P<0.05)。肝功能主要指标两组差异无统计学意义(P>0.05)。结论体外高频热疗联合中医辨证治疗中晚期肝细胞癌在稳定病灶、改善症状、提高生活质量以及延长生存期等方面优于单纯中医辨证治疗,作为一种姑息治疗方案更加适合于不能接受介入治疗、经济无法承受靶向药物的患者群体。更多还原

【Abstract】 Objective To observe the efficacy of external high frequency thermotherapy combining traditional Chinese medicine(TCM) differentiation on relieving the symptoms,prolonging the survival time,improving quality of life,improving liver function in hepatocellular carcinoma.Methods The study adopted a prospective,randomized controlled method.Seventy patients with advanced hepatocellular carcinoma were adopted from September 2008 to September 2009,which were divided into the combining therapeutic arm and TCM differentiation regimen arm according to a table of random digit.The combining therapeutic arm include 37 patients(male:26,female:11).The other arm include 33 patients(male:22,female:11).The two groups were evaluated in the efficacy,survival time,quality of life and liver function after 6 weeks.Results There was little difference on sex,age and stage of the disease(P>0.05) between two arms.After different treatments,the rate of focus stabilization was 40.5%,and 22 cases underwent progressive disease(59.5%) in the therapy arm,while the control arm was 27.3%,and 24 cases underwent progressive disease(73.7%),There was significant difference according to statistical comparisons(P<0.05).For treatment group and control group,median overall survival,time to progress and half-year survival rate were 5.2 months,2.3 months,18.4 percent and 3.7 months,1.6 months,12.6 percent,respectively.There were significant differences(P<0.05).FLIC score were compared after the treatment,the difference has statistical significance(P<0.05).Liver function were compared respectively after the treatment,the difference has no statistical significance(P>0.05).Conclusion The therapy arm obtained better results on relieving the symptoms,prolonging the survival time,improving quality of life,compared with control group.The combining therapeutic regimen can be applied in the clinical as a pauiative treatment in advanced hepatocellular carcinoma patients who can not receive other treatments.更多还原
