【摘要】 目的探讨磁共振T1高分辨力各向同性容积激发(THRIVE)多期动态增强扫描序列在评价门静脉栓塞中的临床价值。资料与方法对48例经病理或临床证实的门静脉栓塞患者(其中癌栓39例、血栓9例)采用常规MR平扫序列和THRIVE多期动态增强序列扫描,评价THRIVE多期动态增强扫描序列对门静脉栓子的显示及定性能力。结果结合常规MR平扫序列,THRIVE多期动态增强扫描对39例癌栓及9例血栓诊断均正确,其对门静脉栓子的定性准确性达100%。48例THRIVE多期动态增强扫描的原始资料经多平面重组(MPR)、最大密度投影(MIP)等方法重组后均能清晰显示门静脉栓子的形态、部位、栓塞程度及门静脉海绵样变、动静脉瘘等并发症情况。结论磁共振THRIVE多期动态增强扫描序列在评价门静脉栓塞中有重要的临床价值。更多还原

【Abstract】 Objective To evaluate the clinical value of MR imaging with multi-phase dynamic contrast-enhanced T1 high resolution isotropic volume examination(THRIVE) in the diagnosis of portal vein embolus.Materials and Meth-ods Firty-eight cases of portal vein embolus(39 cases of cancer emboli and 9 cases of thrombus) confirmed by pathology or clinical data were examined with conventional two-dimensional non-enhanced MR sequence and THRIVE sequence of multi-phase dynamic contrast-enhanced scans.The detection capability of portal vein emboli and its characterization were evaluated for THRIVE sequence of multi-phase dynamic contrast-enhanced scans.Results Combined with conventional non-enhanced MR scan sequence,39 cases of cancer emboli and 9 cases of thrombus were diagnosed correctly in THRIVE sequence of multi-phase dynamic contrast-enhanced scans.The qualitative accuracy of the portal vein emboli was up to 100% in THRIVE sequence of multi-phase dynamic contrast-enhanced scans.Multi-plane reconstructin and maximum intensity projection images of 48 cases could distinctly reveal the location,shape and extent of portal vein embolus,and the complications such as cavernous transformation and arteriovenous fistula.Conclusion THRIVE sequence of multi-phase dynamic contrast-enhanced scans has important clinical value in the evaluation of portal vein embolus.更多还原
