原发性高血压病与颈动脉粥样斑块关系的超声研究

【摘要】 目的探讨各级高血压病与颈动脉粥样斑块形成之间的关系。方法选择Ⅰ、Ⅱ、Ⅲ级极高危高血压病患者(高血压病组)及健康者(对照组)各30例,超声测量各组患者颈动脉内-中膜厚度,观察其有无粥样斑块,并分析斑块发生的部位、数目、形态、内部回声及性质。结果Ⅰ级、Ⅱ级、Ⅲ级极高危高血压病组颈动脉斑块检出率分别为53.33%、56.67%、63.33%,较对照组(16.67%)增加,差异有统计学意义(P<0.05),且高血压病各组间随着分级的增加,斑块检出率增高,但差异无统计学意义(P>0.05)。高血压病组及对照组粥样斑块主要发生在颈总动脉分叉处,其次是颈总动脉、颈内动脉,发生率最低的是颈外动脉,差异有统计学意义(P<0.05)。结论颈动脉粥样斑块的发生率随着高血压分级的增加有增高的趋势;超声能检出颈动脉粥样斑块,并对斑块进行形态学评价,可估计其稳定性。

【Abstract】 Objective To explore the relationship between hypertension grade and atheromatous plaque of carotid arteries.Methods Thirty patients with grade Ⅰ,Ⅱ,Ⅲ extremely high risk hypertension and 30 normal control subjects were enrolled in this study. The intra-media thickness,and the position,number,appearance,internal echo,as well as character of plaques were measured in both sides of carotid arteries by high-frequency ultrasonography.Results The detection rate of plaques in carotid arteries in each hypertension group was 53.33%,56.67%,63.33%,respectively,which was higher than that in control group(16.67%),there was significant difference (P<0.05). With the increase of hypertension grade,the detection rate of plaque was increased,while there was no significant difference (P>0.05). The positions of atheromatous plaque in hypertension and control groups were mainly seen at the bifurcation of common carotid artery,followed by common carotid artery,internal carotid artery,and external carotid artery (P<0. 05).Conclusion The incidence of atheromatous plaque in carotid arteries is increased with the increase of hypertension grade. High-resolution ultrasound can detect the atheromatous plaque of carotid arteries,evaluate the morphology of plaque,and estimate the stability of plaque.
