我院2007—2008年金黄色葡萄球菌耐药性分析

【摘要】 目的了解我院2007—2008年临床分离金葡菌对各类抗菌药物的耐药性。方法按统一方案、采用统一的材料、方法和判断标准(CLSI2006年)对我院从2007年1月—2008年12月分离的金葡菌行耐药性监测。结果我院2007—2008年住院患者金葡菌临床分离株共281株,127株为MRSA,占45.2%;2年的MRSA分离率差异无统计学意义;随着年龄的增加MRSA感染者增多;MRSA对青霉素、头孢唑林、克林霉素、庆大霉素、妥布霉素、红霉素、四环素、磷霉素、环丙沙星、左氧氟沙星、莫西沙星和利福平的耐药率均高于MSSA,差异有统计学意义;对磺胺甲口恶唑-甲氧苄啶的敏感率两者差异无统计学意义;MRSA对万古霉素、替考拉宁、利奈唑胺和奎奴普丁-达福普汀的敏感率仍保持为100%;未发现万古霉素中介株(VISA)和耐药株(VRSA)。2008年分离的金葡菌对莫西沙星的耐药率显著升高。结论MRSA对多种抗菌药物耐药率高,应规范临床用药,加强MRSA耐药性监测,隔离MRSA感染者,防止医源性传播。

【Abstract】 Objective To investigate the antibiotic resistance of S.aureus strains collected in our hospital during 2007 and 2008.Methods All S.aureus strains were isolated from patients during the period from January 2007 to December 2008 in our hospital.Susceptibility testing was carryied out by means of a unified protocol.Results were analyzed according to CLSI 2006 interpretation criteria.Results Of the 281 clinical isolates of S.aureus,127(45.2%) were resistant to methicillin(MRSA).The prevalence of MRSA did not show significant difference between the 2 years.The infection rate of MRSA was increasing with age.The MRSA showed significantly higher resistance to penicillin,cephazolin,clindamycin,gentamicin,tobramycin,erythromycin,tetracycline,fosfomycin,ciprofloxacin,levofloxacin,moxifloxacin and rifampicin than methicillin susceptible S.aureus(MSSA).Howerver,the susceptibility to sulfamethoxazole-trimethoprim showed no significant difference between MRSA and MSSA.MRSA strains were still 100% susceptible to vancomycin,teicoplanin,linezolid,and quinupristin-dalfopristin.No isolate was found nonsusceptible to vancomycin.The resistance rate to moxifloxacin went up significantly.Conclusions MRSA is still highly resistant to many antibiotics.It is essential to strengthen the isolation measures and make reasonable use of antibiotics in order to control MRSA infections.
