【摘要】 目的观察不同输血量对患者出凝血功能的影响,探讨相应的输血对策。方法对185例普通失血患者(A组)和56例容易发生凝血功能障碍的患者(B组)进行输血前后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fib)、D-二聚体(D-D)、血小板计数(PLT)测定。结果 A1组(输血量小于或等于6 U)的患者输血前后凝血功能无明显变化(P>0.05),A3组(输血量大于或等于12 U)的患者输血后比输血前有明显变化(P<0.01)。B1组(输血量小于或等于6 U)的患者与A1组相比,输血后凝血功能有变化(P<0.01)。结论普通失血患者输血量大于或等于6 U,易发生凝血功能障碍的患者输血量大于或等于3 U时,可结合凝血功能检查结果和患者临床症状适当补充新鲜冰冻血浆、浓缩血小板和冷沉淀。更多还原

【Abstract】 Objective To observe the effects of different volume of blood transfusion on the coagulation function and to research into the corresponding transfusion countermeasures.Methods We collected 185 common patients with blood loss(group A) and 56 patients easily occuring coagulation dysfunction(group B).The prothrombin time(PT),activated partial thromboplastin time(APTT),Fibrinogen(Fib),D-dimmer(D-D) and platelet count(Plt) were detected respectively before and after blood transfusion.Results There were no obvious differences in the coagulation function before and after transfusion volume≤ 6 units RBC in group A(P>0.05),but significant differences existed when the blood transfusion volume ≥ 12 units RBC(P<0.01).The evident change of blood coagulation function was found in the patients with transfusion volume ≤ 6 units RBC in group B compared with group A(P<0.01).Conclusion Common hemorrhagic patients with transfusion volume ≥6 units RBC and easily ocuuring coagulation dysfunction,when transfusion volume ≥ 3 units RBC,could be properly replenished with fresh freezing plasma(FFP),platelet concentrate(PC) and cryoprecipitate according to the detection results of coagulation function and the clinical symptoms.更多还原
