【摘要】 目的:探讨低位直肠癌侧方淋巴结清扫的新途径。方法:采取腹膜外入路显露髂组血管后,直视下清扫髂内淋巴结,闭孔淋巴结及骼内动脉外侧相其他淋巴结。结果:50例患者侧方淋巴结得到彻底清扫,髂总动脉下段,髂内、外血管及闭孔神经完全"骨骼化",闭孔显露完整、清晰。结论:低位直肠癌按TME原则手术进入腹腔之前,先经腹膜外清扫侧方淋巴结,盆腔植物神经得以很好保护,清扫彻底,省时、无出血。更多还原

【Abstract】 Objective To investigate the new methods of lateral lymph node dissection for lower rectal cancer.Method After revealing the iliac blood vessels by extraperitoneal approach,the internal iliac lymph nodes,obturator lymph nodes,and relative lateral iliac artery lymph node were dissected under direct vision.Results Thoroughly lateral lymph node dissection in the 50 patients,lower segment of common iliac artery,inside and outside iliac vessels,the blood vessels and obturator nerve were completely "skeletonized",with complete and clear reveal of obturator.Conclusion According to the principles of TME in low rectal cancer surgery,performing lateral lymph node dissection in extraperitoneal approach is prior to opening the abdominal cavity,pelvic autonomic nerves can be well protected,dissection can be thoroughly,also with the advantages of time-saving,non-bleeding.更多还原
