半导体激光配合神经内窥镜治疗颅内蛛网膜囊肿
Endoscopic Neurosurgery Combined with Semiconductor Laser for Treatment of Intracranial Arachnoid Cyst
【摘要】 目的探讨半导体激光配合神经内窥镜在手术治疗颅内蛛网膜囊肿中的应用价值。方法采用半导体激光配合神经内镜手术,选择适当的输出功率,凝固止血时采用功率5~12W,气化切割时采用功率15~30W,进行凝固、止血、气化、切割等手术操作治疗颅内蛛网膜囊肿。结果随访35例,随访时间6~12个月,均行头部CT或MR I动态观察,34例术前症状明显得到改善,1例无效。术后并发硬膜下血肿1例,蛛网膜下腔出血2例,行保守治疗治愈,无复发,无颅内感染等其他并发症,总有效率达97.1%。结论半导体激光配合内窥镜技术治疗颅内蛛网膜囊肿创伤小,术后并发症少,安全,疗效确切。
【Abstract】 Objective To investigate the Curative effects of endoscopic neurosurgery combined with semiconductor laser for the tretment of intracranial arachnoid cyst.Methods The patients with intracranial arachnoid cyst were treated by adopting simple endoscopic neurosurgery,adjusting optical fiber transits porous channel to the diseased region and selecting adequate output power for operative procedure such as coagulate and hematischesis(5~12W)、gasification and incision(15~30W).Results After 6~12 months,35 cases have been followed up.Through head CT/MRI dynamic observation,34 cases with intracranial arachnoid cyst get satisfiable operative effectiveness,were improved distinctly compared to postoperation,but 1 case were no effective.After operation,1 case of epidural hematoma and 2 cases of subarachnoid hemorrhage was healed by conservative treatment.No recurrence,no intracranial infection and other complications.Total effective rate is 97.1%.Conclusion It is safe and effective for endoscopic neurosurgery combined with semiconductor laser to treat intracranial arachnoid cyst and has less trauma and less complications.
