【摘要】 目的分析老年非小细胞肺癌患者的放疗疗效及耐受性,并探讨影响患者生存率的因素。方法回顾性分析73例≥70岁老年非小细胞肺癌放疗患者,接受三维适形放疗45例,常规放疗28例。结果近期疗效:有效率32.9%,完全缓解5.5%(4/73),部分缓解27.4%(20/73),病灶稳定64.4%(47/73)。远期疗效:生存时间3~64个月,中位生存期13个月,1、2年生存率分别为:53.9%、13.8%。副反应:3级放射性肺损伤2例(2.7%),2级以下放射性食管损伤33例(45.2%)。血液毒性反应3级5例(6.8%),4级2例(2.7%)。单因素生存分析显示治疗前KPS≥80,放疗剂量≥60 Gy,早期病变(Ⅰ~Ⅱ期)有统计学意义。结论放射治疗老年非小细胞肺癌的临床疗效可靠且毒副反应小。KPS评分、放疗剂量及临床分期对生存率有影响。更多还原

【Abstract】 Objective To investigate the effectiveness and tolerance of radiotherapy on non-small cell lung cancer(NSCLC) in the elderly patients,and to further study the impact factors of radiotherapy for survival rate.Methods Seventy eligible patients aged no less than 70 with NSCLC were retrospectively studied.Of 73 patients,45 were treated with three-dimensional conformal radiotherapy,28 with conventional radiotherapy.Results Curative effect: total response rate was 32.9%(24/73),complete response rate was 5.5%(4/73),partial remission rate was 27.4%(20/73).Long-term effect: the survival times were from 3 to 64 months;The median survival was 13 months.The 1-year and 2-year survival rates were 53.9% and 13.8%,respectively.Side effects: 2(2.7%) patients had grade 3 lung injury,33(45.2%) patients had degrade 2 esophageal injury.Blood toxicity grade 3 was seen in 5(6.8%) patients,and grade 4 in 2(2.7%).Univariate survival analysis showed that baseline KPS score≥80,radiotherapy dose≥60 Gy and the early stage had statistical significance.Conclusion Radiotherapy has beneficial-effects for elderly patients with NSCLC,but tolerant side effects.The baseline KPS score,radiation dose and clinical stage have significant impact on the survival rate.更多还原
