新生儿缺氧缺血性脑病脑损伤的超声声像图研究

【摘要】 目的探讨超声诊断在新生儿缺氧缺血性脑病(HIE)各种脑损伤中的应用价值。方法选择经临床确诊的不同程度的HIE足月儿37例和有明确窒息史的早产儿15例,以前囟为声窗进行冠、矢状面超声检查和复查。结果早期脑损伤足月儿以脑水肿为主,早产儿以白质病变为主;合并颅内出血足月儿以脉络丛出血为主,早产儿以室管膜下出血为主。晚期脑损伤足月儿以脑实质内囊腔样改变和脑萎缩改变为主,早产儿以脑室周围白质软化为主。结论①超声诊断能较好地反映HIE脑损伤部位及类型,动态观察脑损伤的变化过程。②患儿影像诊断中出现脑萎缩、脑内囊腔样改变、梗阻性脑积水、脑室旁白质软化预示产生后遗症。

【Abstract】 Objective To study the application of ultrasound diagnosis of various brain injury caused by hypoxic-ischemic encephalopathy(HIE).Methods The cranial ultrasonic image of 37 term infants with HIE and 15 preterm infants with asphyxia were recorded,and the exam through prefonticulus in coronal plane and sagittal plane were taken.Results Term infants with brain injury in earlier period mainly had cerebral edema,while preterm infants mainly had cerebral white matter injuries;term infants with intracrinial hemorrhage mainly presented as hemorrhage in choroid plexus while preterm infants mainly presented as subependymal hemorrhage.Brain injury in late period term infants mainly had cystoid alteration and atrophy in brain substance while preterm infants had mainly white matter malacia surrounding the brain ventricle.Conclusion①Cranial ultrasound could reflect the site and type of brain injury in HIE,and could observe the developing process of brain injury dynamically.②Brain atrophy,intracranial cystoid alteration,obstructive hydrocephalus after intracranial hemorrhage,and paraventricle white matter malacia in the image of HIE patients indicate bad sequela.
