【摘要】 目的探讨下肢深静脉血栓形成的临床诊治措施。方法对本院2007年8月~2009年3月收治的经多普勒超声确诊的30例下肢深静脉血栓进行回顾性分析,30例患者均采用负荷剂量加中-大剂量尿激酶12 h连续静点进行溶栓治疗,采用多普勒超声评价治疗效果,观察出凝血指标。结果下肢深静脉血栓形成临床表现消失或明显减轻。多普勒超声复查显示血栓全部或部分消失。治疗结束时血浆D-二聚体、凝血酶原时间(PT)明显升高(分别为P<0.05和P<0.01),纤维蛋白原较治疗前明显下降(P<0.01),随访3~8月,除1例自行停用华法林复发外,余均无复发。结论溶栓治疗是下肢深静脉血栓形成的基础疗法之一。负荷剂量加中-大剂量持续静点尿激酶可有效溶解血栓,特别是对病史较长者具有较好的临床疗效,且不增加出血风险。更多还原

【Abstract】 Objective To investigate the therapeutic effect of loading dose followed by large-dose urokinase on deep vein thrombosis of lower limbs.Methods Thirty cases that were diagnosed with deep vein thrombosis of lower limbs by doppler ultrasound were analyzed.All patients were treated with continuous large-dose urokinase for 12 h and the therapeutic effect was estimated with doppler ultrasound.D-dimer and prothrombin time(PT) were also observed.Results The clinical manifestation of deep vein thrombosis of lower limbs totally or partly disappeared.Doppler ultrasound showed that the thrombus totally or partly disappeared.D-dimer and PT obviously increased(P<0.05 and P<0.01 respectively),while fibrinogen(FIB) significantly decreased(P<0.01)compared with those before treatment.After 3~8 months follow-up,all cases did not reoccur,except one who stopped using warfarin.Conclusion Thromblysis is the basic treatment of deep vein thrombosis of lower limbs and loading dose followed by continuous large-dose urokinase has good therapeutic effect on it.更多还原
