【摘要】 目的:探讨发生于涎腺的透明细胞肌上皮癌和非特异性透明细胞癌的组织病理与免疫组化特征。方法:对3例透明细胞肌上皮癌和5例非特异性透明细胞癌进行常规HE染色和免疫组化(CK-AE1/AE3,S100,vimentin,smooth muscle actin,calponin,P63和maspin),并对结果进行比较。结果:两种肿瘤都几乎全部由透明细胞构成,在透明细胞肌上皮癌中可见少量的梭形细胞,在非特异性透明细胞癌中可见少量含有嗜酸性胞浆的细胞。免疫组化显示:透明细胞肌上皮癌中,3例均阳性表达CKAE1/AE3,S100,vimentin,P63和maspin,2例分别阳性表达smooth muscle actin和Calponin;非特异性透明细胞癌中,5例均阳性表达CKAE1/AE3,P63和maspin,而S100,vimentin,smooth mus-cle actin,calponin均呈阴性表达。结论:涎腺透明细胞肌上皮癌和非特异性透明细胞癌都很罕见,二者可以通过组织病理和免疫组化特点进行鉴别。由于肿瘤性肌上皮细胞可呈现不同的肌源性分化,因此,联合运用多种提示肌上皮源性的标志物如CKAE1/AE3,S100,vimentin,smooth muscle actin和calponin对二者鉴别诊断有帮助。P63和maspin缺乏特异性。更多还原

【Abstract】 Objective: To evaluate the histopathological and immunohistochemical characteristics of 3 cases with clear cell myoepithelial carcinoma and 5 cases with non-specific clear cell carcinoma in salivary gland.Methods:Date of 3 cases with clear cell myoepithelial carcinoma and 5 cases with clear cell carcinoma of non-specific were analyzed retrospectively.The conventional HE dyeing and the immunohistochemistry(CKAE1/AE3,S100,vimentin,smooth muscle actin,calponin,p63 and maspin)were used for the comparative observation.Results:Both of the two kinds of tumors were almost entirely composed of clear cells.A few of spindle cells could be observed in the clear cell myoepithelial carcinoma,while a few of cells containing eosinophilic cytoplasm could be oberseved in the non-specific clear cell carcinoma.Immunohistochemistry showed that the CKAE1/AE3,P63 and maspin were expressed masculinely,but S100,vimentin,smooth muscle actin and calponin were expressed negatively in the three clear cell myoepithelial carcinoma.Conclusion:Both of salivary clear cell myoepithelial carcinoma and non-specific clear cell carcinoma are very rare.They can be distincted by histological and immunohistochemical characteristics.As the neoplastic myoepithelial cells can show different myogenic differentiation,which prompts the joint use of a variety of markers of myoepithelial-derived,e.g,CKAE1/AE3,S100,vimentin,smooth muscle actin and calponin may assistant for the distinction diagnosis of the two kinds of carcinoma.P63 and maspin lack the specificity.更多还原
