【摘要】 目的探讨大面积脑梗死急性期颅内血管血流动力学变化的特点。方法对43例大面积脑梗死患者在急性期进行TCD检查,观察颅内血管血流动力学变化情况。结果 43例大面积脑梗死患者的TCD显示,大面积脑梗死病侧大脑中动脉(MCA)平均血流速度(Vm)、颈内动脉(ICA)、平均血流速度(Vm)明显低于对照组(P<0.05)和病灶对侧(P<0.05),双侧大脑前动脉(ACA)Vm与对照组比较无显著差异;血管搏动指数和阻力指数(PI、RI)与对照组比较明显增大,差异有统计学意义(P<0.05)。结论大面积脑梗死可通过床边TCD及时了解颅内血流动力改变及血管阻力,从而了解梗死区血供情况,间接了解颅高压、脑水肿程度,为判断病情、预后及外科手术治疗提供参考。更多还原

【Abstract】 Objective To investigate the characteristics of hemodynamics in patients with large area cerebral infarction.Methods Forty-three patients with large area cerebral infarction were enrolled into our study.All the patients were examined by TCD in the acute stage to observe the characteristics of hemodynamics.Results Among 43 patients by TCD,the mean velocity of ipsilateral ICA and MCA were found significantly lower than those of contralateral side (P<0.05) and control group (P<0.05).But no significant difference was found in the ACA between control group and observation group.The PI and RI were found higher in observation group than those in control group (P<0.05).Conclusion TCD can help to estimate the changes of hemodynamic and vascular resistance,and furthermore to know the blood supply of brain tissue and intracranial pressure in patients with massive cerebral infarction.So,these results will be useful in estimating the prognosis of patients and choosing operation.更多还原
