【摘要】 目的比较胃癌合并糖尿病患者不同消化道重建对2型糖尿病的治疗效果。方法 56例胃癌合并糖尿病患者胃癌根治术后采用不同的消化道重建方式(BillorthⅠ式吻合9例;BillorthⅡ式吻合28例;Roux-en-y吻合19例),比较不同重建术后糖尿病的治疗效果。结果 3组术后胰岛素平均用量相比BillorthⅡ组及Roux-en-y组显著低于BillorthⅠ组,Bil-lorthⅠ组、BillorthⅡ组和Roux-en-y组的糖尿病治疗有效率分别为22.2%、67.9%和68.4%,BillorthⅡ组和Roux-en-y组的有效率显著优于BillorthⅠ组。结论胃癌合并糖尿病在胃癌根治术后采用BillorthⅡ或Roux-en-y吻合更可能实现胃癌及糖尿病均好转甚至治愈的愿望。更多还原

【Abstract】 Objective To observe the effect of enteron rebuild after gastrectomy on diabetes mellitus. Methods Analysis the clinical data of 56 gastric carcinoma patients with diabetes mellitus in our hospital. All these patients were devided 3 groups: BillorthⅠ anastomoses group( n = 9) ,BillorthⅡanastomoses group( n = 28) and Roux- en - y anastomoses group( n = 19) . Record the changes in drugs for diabetes before and after surgery. Evaluate the different improvement of diabetes after the three operations. Results The effective rates of diabetes in BillorthⅠgroup is lower than that in BillorthⅡgroup and Roux - en - y group( P < 0. 05) . But there is no evident difference between BillorthⅡgroup and Roux - en - y group( P > 0. 05) Conclusion BillorthⅡand Roux - en - y anastomoses may be better for the gastric carcinoma patients with diabetes mellitus.更多还原
