【摘要】 目的比较原发性急、慢性闭角型青光眼激光虹膜周边切开术前后昼夜眼压波动情况以及视网膜中央动脉血流变化;并探讨激光虹膜周边切开术后视网膜中央动脉血流与眼压波动的关系。方法对26例原发性急性闭角型青光眼临床前期、间歇期患眼,26例原发性慢性闭角型青光眼早期患眼,激光虹膜周边切开术前后分别进行昼夜眼压测量(选择时间点为14:00、18:00、22:00、2:00、7:00、10:00)和视网膜中央动脉血流测量(选择时间为10:00、18:00)。结果患眼6个时间点均有降眼压效果,且各个时点间差异有统计学意义(P<0.05)。原发性急性闭角型青光眼术后视网膜中央动脉血流,10:00,收缩期峰值血流速度(peak systolic velocity,PSV)、舒张末期血流速度(end diastolic velocity,EDV)(P<0.05),阻力指数(resistive index,RI)降低(P<0.05),眼压和RI呈正相关(r=0.85,t=3.726,P<0.05);18:00,PSV增加不明显(P>0.05),EDV增加(P<0.05),RI有所降低(P<0.05),眼压和RI正正相关(r=0.76,t=3.672,P<0.05);原发性慢性闭角型青光眼术后视网膜中央动脉血流,10:00,PSV、EDV均增加(P<0.05),RI有所降低(P<0.05),眼压和RI呈正相关r=0.78,t=3.543,P<0.05);18:00,PSV、EDV均增加(P<0.05),RI降低(P<0.05),眼压和RI呈正相关(r=0.88,t=3.782,P<0.05)。结论对于原发性急性闭角型青光眼临床前期或缓解期以及原发性慢性闭角型青光眼早期行激光虹膜周边切开术术后眼压整体水平下降,眼压波动幅度降低,视网膜中央动脉血流阻力指数也减低,视网膜中央动脉血流灌注增加,血液供应得到改善。更多还原

【Abstract】 Objective To investigate intraocular pressure(IOP) fluctuations and hemodynamics of central retinal artery of patients with acute and chronic primaiy angle - closure glaucoma after laser peripheral iridotomy,and further to explore the correlation between IOP and central retinal arterial blood. Methods The preclinical and remission eyes of 26 cases of acute primary angle - closure glaucoma (APACG) and the affected eyes of 26 cases of chronic primary angle - closure glaucoma (CPACG) received laser peripheral iridotomy.The diurnal IOP fluctuation (Time,14: 00,18: 00,22: 00, 2:00,7:00,and 10: 00) and hemodynamics of central retinal artery (Time,10am and 18: 00 ) were measured Results The laser peripheral iridotomy for patients with acute and chronic angle - closure glaucoma decreased IOP at 6 time points; the reduced IOP levels at the time points were different ( P < 0.05). After laser peripheral iridotomy APACG eyes showed: peak systolic velocity ( PSV), end diastolic velocity (EDV) increased ( P < 0.05 ),and resistance index ( RI) reduced ( P < 0.05),zop were opsitively correlated with RI ( r = 0.85,t = 3.726,P < 0.05) at 10: 00.PSV increased insignificantly ( P >0.05),EDV increased ( P <0.05),and RI reduced ( P <0.05) zop were opsitively correlated with RI( r =0.76,t =3.672,P <0.05)at 18: 00.CPACG eyes showed:PSV, EDV increased ( P <0.05),and RI reduced ( P <0.05),zop were opsitively correlated with RI( r = 0.78,t =3.543,P <0.05)at 10: 00.PSV,EDV increased ( P <0.05),and RI reduced ( P < 0.05 ),zop were opsitively correlated with RI ( r =0.88,t=3.782,P < 0.05) at 18: 00.Conclusion The overall level of IOP drops in acute and chronic primary angle - closure glaucoma after laser peripheral iridotomy.After laser peripheral iridotomy,central retinal arterial perfusion is increased and blood supply is improved.更多还原
