【摘要】 目的分析口腔癌患者T淋巴细胞免疫的特点及手术对口腔癌患者免疫功能的影响。方法选用20例口腔癌患者和15例健康志愿者为试验组和对照组,试验组于手术前、后各1周分2次采取新鲜外周静脉血1mL,对照组采取新鲜外周静脉血1mL,均以肝素抗凝。每份样本取血100μL,分别加入CD3、CD4、CD8、CD56单克隆抗体10μL,检测T淋巴细胞各表型占淋巴细胞总量的百分比,并计算CD4/CD8的比值。再分取抗凝静脉血100μL,分别加入10μL白细胞介素-2(interleukin-2,IL-2)、CD4、干扰素-γ(interferon-γ,INF-γ)单克隆抗体,检测IL-2阳性,INF-γ阳性,IL-2、CD4双阳性及INF-γ、CD4双阳性T细胞占总细胞的百分比。结果试验组手术前1周与对照组比较CD3、CD4的表达降低,CD8的表达增加,CD4/CD8比值下降(P<0.05);术后1周CD4的表达比术前有了显著的提高,CD8的表达下降,CD4/CD8比值较术前有所回升(P<0.05)。试验组术前淋巴细胞内各细胞因子含量较对照组均明显下降,术后各因子水平有了不同程度的回复。结论口腔癌患者体内抗肿瘤免疫处于抑制状态,手术对其有一定的改善作用,但仍有必要配合免疫治疗。更多还原

【Abstract】 Objective To analyze the characteristics of the phenotype of T lymphocytes and the effect of surgery on the immune status of patients with oral carcinoma,then provide the theoretical basis for the application of immunotherapy on oral carcinoma.Methods Twenty cases of oral cancer patients and fifteen healthy volunteers were selected as experimental group and control group.1mL peripheral blood was taken from each subject on the days one week before and after operation.All the blood were processed by heparin.Then 10μL CD3,CD4,CD8,CD56 monoclonal antibody was added into 100μL anticoagulant blood respectively.Red blood cell lysis method was used to detect the phenotype of T lymphocytes and the percentage of each phenotype T lymphocytes to the total lymphocyte and the ratio of CD4 to CD8 was also calculated.Another 100μL anticoagulant blood was taken from each sample.And 10μL interleukin - 2 ( IL - 2 ),CD4,interferon -γ( INF -γ) monoclonal antibody was added respectively to detect the characteristics of the phenotype of T lymphocytes and the effect of surgery on the immune status of patients with oral carcinoma. Results The expression of lymphocyte phenotype one week before surgery in the peripheral blood of patients was different from control groups.The expression of CD8 was increased while CD3,CD4 were reduced and the ratio of CD4/CD8 was decreased ( P < 0.05); However one week after surgery,the expression of CD8 was decreased while CD4 improved significantly and the ratio of CD4/CD8 was increased ( P <0.05).The levels of lymphocyte cytokine in pre - operative oral cancer patients were significantly decreased compared with the control group and they were recoverd partially after surgery.Conclusion The immune state of patients with oral carcinoma are surpressed before surgery and can be improved by operation,but can not be fully restored to the level of healthy control group,it is necessary to apply immune therapy to enhance the level of anti - tumor immunity of patients with oral carcinoma.更多还原
