耻骨联合分离程度对髋臼与股骨头之间应力影响的研究
INFLUNENCE OF SYMPHYSIS PUBIS DIASTASIS ON THE PRESSURE BETWEEN THE ACETABULUM AND THE FEMORAL HEAD
【摘要】 目的评价耻骨联合分离程度对髋臼与股骨头之间应力的影响。方法 8具完整骨盆及近侧1/3股骨标本,用夹具将其固定于双足站立骨盆中立位,在LDS-500试验机上加载至体质量的5/6,维持2min。用压敏片依次测量正常骨盆、耻骨联合分离1.0、2.0、2.5、3.0cm时髋臼前壁、后壁和负重顶区的应力和应力分布变化。结果耻骨联合完整时负重顶区应力为(0.71±0.21)MPa,分离2.0cm时为(1.14±0.24)MPa,分离2.5cm时为(1.25±0.28)MPa,分离3.0cm时为(1.31±0.23)MPa,耻骨联合分离≥2.0cm时负重顶区平均应力显著增加(P<0.05)。耻骨联合完整时髋臼前壁应力为(1.03±0.30)MPa,分离2.0cm时为(0.78±0.12)MPa,分离2.5cm时为(0.74±0.15)MPa,分离3.0cm时为(0.72±0.13)MPa,耻骨联合分离≥2.0cm时,前壁的平均应力显著减少(P<0.05),变化最明显的部位是髋臼后壁,耻骨联合完整时应力为(0.81±0.11)MPa,分离2.0cm时为(1.16±0.23)MPa,分离2.5cm时为(1.30±0.31)MPa,分离3.0cm时为(1.48±0.25)MPa,耻骨联合分离≥2.0cm时使后壁的平均应力显著增加(P<0.05)。结论耻骨联合分离≥2.0cm时显著改变了髋臼与股骨头之间的应力和应力分布;变化最显著的部位是髋臼后壁。
【Abstract】 ObjectiveTo evaluate the effect of different degrees of symphysis pubis diastasis on the pressure between the acetabulum and the femoral head.Methods Eight embalmed pelvis and articulated proximal 1/3 of the femurs were harvested for this study.The position of the pelvis and proximal femurs during testing simulated the both leg stance phase of gait.The pressure sensitive films recorded the pressure,and the distribution of the pressure between the acetabulum and the femoral head. Testing was initially performed with the intact symphysis pubis.After the intact patterns were recorded, the diastasis of 1.0cm,2.0cm,2.5cm and 3.0cm were tested.Results After the symphysis pubis diastasis more than 2.0cm,the pressure of the weight -bearing dome was significantly increased ( P < 0.05).The pressure of the anterior wall was significantly decreased ( P <0.05).The pressure of the posterior wall was significantly increased ( P < 0.05 ).Conclusion The pressure between the acetabulum and the femoral head is dramaticly altered when the symphysis pubis diastasis more than 2.0 cm. The pressure of the hip joint is redistributed after the symphysis pubis diastasis. The greatest alteration of joint pressure occur in the posterior wall.
