替奈替普酶诱导兔玻璃体后脱离的实验研究
AN EXPERIMENT STUDY ON TENECTEPLASE INDUCING POSTERIOR VITREOUS DETACHMENT IN RABBITS
【摘要】 目的探讨替奈替普酶(tenecteplase),TNK变异型组织纤维蛋白溶解酶原激活剂(TNK-tissueplasminogen activator,TNK-TPA)在兔眼玻璃体腔注射联合睫状体冷冻诱导玻璃体后脱离的有效性和安全性。方法 30只成年有色兔,排除内外眼病。随机分为3组,每组10只。每只兔任意一眼为实验用药眼,玻璃体腔注入实验药物500 μg/mL替奈替普酶0.1 mL;另一只眼为对照眼,玻璃体腔注入0.1 mL生理盐水。第1组注药后观察1h,第2组注药后观察1 d,第3组注药后观察3 d。通过裂隙灯、双目检眼镜、B型超声、视网膜电图等检查,并且在检查结束后除眼球做光镜、扫描电镜和透射电镜检查,观察药物注入后诱导玻璃体后脱离的情况及药物应用的安全性。结果玻璃体腔注入替奈替普酶后1~3 d,视网膜结构和功能没有任何不良影响;其中,在注药后1d与3d,10只实验眼中分别有8眼和9眼发生了玻璃体后脱离,与对照眼和注药后1h相比差异有统计学意义(P<0.05)。注药1 d与3 d相比差异无统计学意义(P>0.05)。注药1 d和3 d后光镜和扫描电镜发现发生了完全性玻璃体后脱离,注射药物1 h、1 d后透视电镜未发现视网膜变性。结论在破坏血眼屏障后,玻璃体腔内注射替奈替普酶可以诱导玻璃体后脱离的发生。
【Abstract】 Objective To investigate the efficacy and safety for intravitreal injection of tenecteplase combined with cyclocryoapplication on production of posterior vitreous detachment (PVD) in rabbit eyes.Methods Thirty adult colored-rabbits without any eye diseases were used as experimental animals.They were divided into 3 groups randomly,10 rabbits in one group.One eye received an intravitreal injection of tenecteplase(500 μg/mL) after the cyclocryoapplication via conjuctiva as experimental eye;another one received normal saline as control.Group Ⅰ were observed for 1 hour,group Ⅱ were observed for 1 day,group Ⅲ were observed for 3 days.All experimental eyes were examined by slit-lamp biomicroscopy,binocular indirect ophthalmoscopy, 90D double aspheric preset lens,B ultrosonography and eletroretinography (ERG).After clinical observation,the experimental eyes were enucleated to perform light microscope,scanning electron microcope and transmission electron microcope examination.Results During the observing period, tenecteplase (500 μg/mL) was no harmful to the structure or function of the injected eyes;On 1d and 3d postinjection,8 eyes and 9 eyes in 10 eyes of experimentals had evidence of a PVD.The results showed significant differences between the experimental groups(1d group and 3 d group) and control group、1h group.But there was no significant difference between the 1 d group and the 3 d group.The eyes in 1d group and 3 d group,were induced PVD observed by light microscope and scanning electron microscope.There was no degeneration observed in retina in 1 h group and 1 d group by transmission electron micrescope.Conclusion Intravitreal injection of tenecteplase ( 500μg/mL) combined with cyclocryoappliction can induce posterior vitreous detachment in rabbits.
