【摘要】 目的探讨玻璃体切除联合硅油填充术与单纯玻璃体切除术对化脓性眼内炎的疗效。方法将42例化脓性眼内炎随机分成2组,A组(对照组)行单纯玻璃体切除术,B组(观察组)行玻璃体切除联合硅油填充术,2组同时配合全身及局部使用抗生素及皮质激素治疗。术后随访3～18个月,观察术后眼压、视功能、感染控制及再次手术的情况。结果随访3～18个月,玻璃体切除联合硅油填充术较单纯玻璃体切除术有更明显的视力提高(A组26.32%,B组34.78%)、眼压维持(A组1.86 kPa,B组2.21 kPa)及更低的眼内炎复发(A组10.52%,B组0%)、视网膜脱离(A组21.05%,B组4.34%)。2组比较差异有统计学意义(P<0.05,P<0.01)。结论玻璃体切除联合硅油填充术能稳定视网膜功能、控制炎症、减少再次手术引起的创伤及眼球萎缩的机会,是治疗化脓性眼内炎较为有效的方法。更多还原

【Abstract】 Objective To investigate the efficacy of the silicone oil combined vitrectomy surgery and simple vitrctomy alone for endophthalmitis treatment.Methods Fourty-two cases of endophthalmitis were randomly divided into two groups,A group was treated by simple vitrectomy,and B group was treated by vitrectomy with silicone oil injection,the two groups simultaneously with systemic and topical antibiotics and corticosteroid treatment.After 3 to 18 months follow-up,the postoperative intraocular pressure,visual function,inflammation control and the situation of re-operation were observed.Results After 3 to 18 months follow-up,the outcome was better in vitrectomy combined with silicone oil group,including improved visual acuity (A group accounted for 26.32%,B group accounted for 34.78%),more stable intraocular pressure( A group 1.86 kPa,B group 2.21 kPa) ,lower recurrence of endophthalmitis (A group 10.52% ,B group 0%),and lower recurrence of retinal detachment (A group 21.05%,B group 4.34 %).There were statistically significant differences between the two groups ( P <0.05,P < 0.01).Conclusion Vitrectomy combined with silicone oil injection can stablize retinal function, control inflammation,reduce the opportunity of re-operation and eyeball shrinking,and it is an effective treatment of endophthalmitis.更多还原
