高压氧对脑梗死患者Hs-CRP、MMP-9、NSE及预后的影响
The effect of HBO on Hs-CRP,MMP-9,NSE in patients with acute cerebral infarction and Clinical prognosis
【摘要】 目的探讨高压氧(HB0)治疗对急性脑梗死患者超敏C-反应蛋白(Hs-CRP)、基质金属蛋白-9(MMP-9)及神经元特异性烯醇化酶(NSE)水平的影响及其临床意义。方法收集65例颈内动脉系统脑梗死患者,随机分为HBO治疗组(HBO组)34例和常规治疗组(对照组)31例。所有患者住院第1天、第5天和第10天时用采用酶联免疫吸附方法检测患者外周血清MMP-9,胶乳增强免疫比浊法检测血清Hs-CRP,固相酶联免疫法测定血清NSE浓度的动态变化,MRI测定比较各组患者的脑梗死灶体积。并于入院时及住院第10天、30天采用FMA评分对两组患者进行神经功能缺损评分,评定其临床疗效。结果 HBO组和对照组各检测指标治疗前比较,差异无统计学意义(P>0.05);治疗后均有下降,但HBO组明显低于对照组(P<0.05);HBO组梗死体积亦显著小于对照组(P<0.05);与对照组比较,HBO组FMA评分明显增加(P<0,05)。结论 HBO辅助治疗能显著降低脑梗死患者血清Hs-CRP、MMP-9和NSE的水平,缩小梗死灶体积,可有效促进其神经细胞功能恢复,提高临床疗效。HBO可能通过影响Hs-CRP、MMP-9的表达发挥治疗作用。
【Abstract】 Objective To search for the effects of hyperbaric oxygen therapy on Hypersensitive C- Reactive Protein (Hs-CRP),matrix metalloproteinase-9 (MMP-9),Neuron-specific enolase (NSE) in patients with acute cerebral infarction and clinical prognosis.Methods 65 patients with acute cerebral infarction in the carotid artery system were divided into two groups randomly which were HBO group and control group.The HBO group (n=34) were treated with HBO therapy in addition to routine clinical treatment The control group (n=31) were treated with routine clinical treatment regime.The serum concentrations of MMP-9,NSE were measured with ELISA,Hs-CRP were measured with Latex-enhanced Immunonephelometry,NSE were measured with Solid-phase Enzyme Immunoassay on the 1st,5th and 10th day,The volumes of cerebral infarction and its peripheral ededma were evaluated by MRI.The assessment of neurological deficits was conducted with FMA score before treatment,and at 10 and 30 days after treatment,and the therapeutic efficacy was evaluated.ResultsThe serum concentrations of Hs-CRP,MMP-9,NSE were all decreased after treatment in the two patient groups. But these parameters of HBO groups were significantly lower than that of control group (P<0.05). The volumes of cerebral infarction and its peripheral edema of HBO group were lesser than that of control group (P<0.05).The FMA scores of HBO group were significantly higher than that of the control group (P<0.05).Conclusion HBO therapy can reduce the volumes of cerebral infarction and its peripheral edema,encourage neurocyte function recovery,improve the rapertic effect.Decreasing the serum levels of Hs-CRP,MMP-9 and NSE might be the mechanisms of HBO in the treatment of cerebral infarction.
