蛇毒降纤酶治疗慢性青光眼的研究

【摘要】 目的:探讨蛇毒降纤酶降低青光眼患者血黏度、改善眼局部微循环以及对青光眼的治疗作用。方法:将100例抗青光眼术后眼压控制的中、晚期青光眼患者随机分为实验组和对照组。①实验组:应用蛇毒降纤酶进行治疗。于治疗前行眼压、血浆纤维蛋白原含量、血液流变学、眼血流动力学检测及电脑视野检查,治疗后第1个月、3个月及6个月分别复查上述各项指标并作记录;②对照组:不采用任何药物,参照上述相应时间及各项指标进行检测并作记录。结果:两组青光眼治疗前血浆纤维蛋白原、血液流变学及血流动力学各指标经统计学处理,差异均无统计学意义(P>0.05);实验组治疗后1、3个月上述各项指标与治疗前比较,差异均有统计学意义(P<0.05),治疗后6个月差异均无统计学意义(P>0.05)。实验组治疗后1个月、3个月,电脑视野的MD值与对照组比较有明显进步(P<0.05);治疗后6个月又基本恢复到治疗前水平。结论:①蛇毒降纤酶具有明显降低慢性青光眼患者血黏度、改善眼微循环的治疗作用;②视神经血流灌注量对青光眼的视功能损害可能起一定作用;③蛇毒降纤酶对慢性青光眼及视神经缺血病变的临床治疗有很好的应用价值。

【Abstract】 Objective:To evaluate the effect of defibrase on reduction of blood viscosity,improvement of ocular microcirculation and treatment of glaucoma.Methods:One hundred advanced glaucoma patients after surgery intraocular pressure <2.80 kPa(drug or no drugs)were randomly divided into experimental group and control group.The experimental group(treatment defibrase):the administration of defibrase.Intraocular pressure,plasma fibrinogen,hemorheology,ocular hemodynamics and computer perimetry testing were examined before and 1,3 and 6 months after treatment respectively.The above-mentioned target for record was reviewed.The control group(control)don’t use any drugs.The same target was detectd and recorded.Results:Plasma fibrinogen,blood rheology and hemodynamics in two groups before treatment was no statistically significant difference(P>0.05).Experimental group 1 month and 3 months after treatment compared with the target pre-treatment,the differences were statistically significant(P<0.05).There was no statistically significant difference(P>0.05)after 6 months.Chronic glaucoma after defibrase treatment 1 and 3 months,the MD value of computer vision compared with the control group there is marked improvement(P<0.05);months 6 after treatment to return to the level before treatment.Conclusion:Defibrase venom can decrease blood viscosity,improve ocular microcirculation for chronic glaucoma patients.Optic nerve blood flow volume of the visual function of glaucoma damage may play a role.Defibrase venom has a very good value for the clinical treatment of chronic glaucoma/optic nerve ischemia.
