FISH技术检测宫颈上皮细胞中hTERC基因扩增的临床研究

【摘要】 目的:应用荧光原位杂交技术(FISH)检测人类染色体末端酶hTERC在宫颈上皮脱落细胞中的扩增,研究hTERC基因在不同程度宫颈病变的表达及临床意义。方法:采用TCT低渗制片法,FISH检测15例宫颈良性病变、27例宫颈上皮内瘤变(CINⅠ14例、CINⅡ8例、CINⅢ5例)、37例浸润性子宫颈鳞癌(SC)、4例子宫颈腺癌以及13例人乳头瘤病毒(HPV)阳性妇女的宫颈脱落细胞中hTERC基因的表达。结果:FISH检测宫颈上皮脱落细胞hTERC基因异常扩增率43.62%(41/94);在宫颈良性病变、宫颈上皮内瘤变、SC、宫颈腺癌以及HPV阳性各组中发生hTERC基因异常扩增的百分比例分别为6.67%、11.11%、83.78%、100%、15.38%;CIN组hTERC基因异常中CINⅠ、CINⅡ、CINⅢ(包括原位癌)分别占7.69%、12.5%、20.0%,差异有统计学意义(P<0.05)。结论:hTERC基因扩增随宫颈疾病程度加重递增,CIN组中随级别增高其阳性表达率亦明显增加;FISH检测hTERC基因异常扩增可能有助于早期宫颈疾病的筛查,并有望对宫颈病变的进展及评估预后等提供新方法。

【Abstract】 Objective:Fluorescence in situ hybridization(FISH)technology was used to detect the human telomerase gene(hTERC)in epidermis cervixcl cast-off cells,and to research the expression of hTERC in cervix lesion in various degree and the clinical significance.Methods:The expression of hTERC in epidermis cervixcl fall-off cells was examined by TCT low seep methode and fluorescence in situ hybridization(FISH)technology in 15 cases of benign cellular changes,27 cases of cervical intraepithelial neoplasia internal(CIN,14 cases of CINⅠ,8 cases of CINⅡ,5 cases of CINⅢ),37 cases of squamous-carcinoma(SC),4 cases of cervix adenocarcinoma,and 13 cases of HPV musculine.Results:The musculine rate is 43.62%(41/94)when FISH was used to detect epidermis cervixcl fall-off cells.The percentages of gene exceptional augmentation in hTERC of benign cellular changes,CIN,SC,cervixc adenocarcinoma and HPV musculine were 6.67%,11.11%,83.78%,100%,15.38%;respectively.The percentages of hTERC exceptional in group CINⅠ,CINⅡ and CINⅢ were 7.69%,12.5% and 20.0%(P<0.05);respectivety.Conclusion:hTERC has become increasing more serious comply with cervixc disease,the musculine expresion rate was obvious to raise in random level in group CIN;FISH detect gene exceptional augmentation in hTERC was helpful in search early period cervix disease,and will provide new methods for making headway and evaluating prognosis in cervix disease.
