【摘要】 目的了解自身免疫性肝病(ALD)患者可能出现的各种自身抗体及其检出率,提高对该病的诊断水平。方法采用间接免疫荧光法和欧蒙印迹法检测122例临床确诊的ALD患者、100例健康人及100例病毒性肝炎患者的抗核抗体(ANA)、抗平滑肌抗体(ASMA)、抗线粒体抗体(AMA)、抗线粒体Ⅱ型抗体(AMA-M2)、抗肝肾微粒体抗体Ⅰ型(LKM-1)、抗可溶性肝抗原/肝胰抗原(SLA/LP)抗体。122例自身免疫性肝病患者分为3组:自身免疫性肝炎组(AIH)31例,原发性胆汁性肝硬化(PBC)组83例,原发性硬化性胆管炎(PSC)组8例。结果 AIH组ANA、ASMA、抗LKM-1、抗SLA/LP的阳性率分别为80.65%、45.16%、12.90%、16.13%;PBC组ANA、AMA-M2阳性率为84.34%、89.16%;PSC组检测到ANA阳性1例(12.50%)。结论大多数ALD伴有特异性自身抗体的出现;ALD具有高滴度的自身抗体水平,而病毒性肝炎诱导的自身抗体水平滴度较低;自身抗体的检测对临床肝病学具有重要的意义。更多还原

【Abstract】 Objective To know the positive rates of various autoantibodies probably existed in patients with autoimmune liver disease and to improve its recognition.Methods 122 patients were divided into three groups,among which 31 patients with autoimmune hepatitis,83 patients with primary biliary cirrhosis,8 patients with primary sclerosing cholangitis.122 patients with autoimmune liver disease and 100 patients with hepatitis B or C.And 100 healthy people were respectively detected by indirect immunefluorescence and immuoblot assay about antinuclear antibodies(ANA),antismooth muscle antibodies(SMA),antimitochondrial antibodies(AMA),subtype of AMA(M2),anti-liver-kidney microsomal antibody type 1(LKM-1),soluble liver antigen/liver pancreas(SLA/LP).Results Positive rates of ANA,SMA,LKM-1 and SLA/LP were 80.65%、45.16%、12.90%、16.13% respectively in autoimmune hepatitis.ANA、AMA-M2 showed positive rates of 84.34%、73.49% respectively in primary biliary cirrhosis.Positive ANA was detected in 1case with primary sclerosing cholangitis(12.5%).Conclusion The majority of autoimmune liver diseases was accompanied with specific autoantibodies.The detection of autoantibodies is important in clinical hepatology.更多还原
