FISH检测子宫颈脱落细胞hTERC基因的表达及意义
Expression of the human telomerase gene in exfoliative cytes of cervix
【摘要】 目的:探讨人类3号染色体端粒酶(hTERC)基因在宫颈上皮内瘤变(CIN)患者的表达及临床意义。方法:收集2008年1月至2008年10月东莞地区100例宫颈脱落细胞标本,其中CIN患者63例、宫颈鳞癌(SCC)患者7例、正常细胞学或炎症妇女30例,用荧光原位杂交(FISH)方法检测宫颈脱落细胞hTERC基因。结果:在CINⅠ、CINⅡ/Ⅲ和SCC患者宫颈脱落细胞中hTERC基因的表达率分别是15.15%、89.67%和100%,CINⅠ、CINⅡ/Ⅲ和SCC组与正常组比较,hTERC基因阳性率差异有显著统计学意义(P<0.001),其中,CINⅠ与CINⅡ/Ⅲ比较,CINⅡ/Ⅲ与SCC比较差异有统计学意义(P<0.05)。随着病变程度增加,hTERC基因表达率增加。hTERC基因检测CINⅡ/Ⅲ的敏感度、特异度、阳性预测值和阴性预测值分别为86.67%、92.06%、83.87%和93.53%。CIN和SCC组病例与正常或炎症组比较,hTERC基因扩增比例增加,可观察到明确的超二倍体现象。hTERC基因的表达水平与宫颈病变的程度关系密切。结论:hTERC基因在CIN和SCC中表达异常,随着病变程度增加其阳性率增加,可作为宫颈病变恶性进展的监测标志。
【Abstract】 Objective:To evaluate the significance of expressing the human telomerase gene(hTERC)in the exfoliative cytes of cervix by dual-color interphase fluorescence in situ hybridization(FISH). Methods:The fluorescence signal in the cells were detected by using interphase FISH.We collected 100 specimens in Dongguang from January to October of 2008 and detected the expression of hTERC gene in cells from 63 cervical intraepthelial neoplasia(CIN),7 squamous carcinomas of the cervix(SSC) and 30 nomal cervix or chronic cervicitis. Results:Positive expression rates of the hTERC gene in CIN Ⅰ、CINⅡ/Ⅲ and SSC were 15.15%,89.67% and 100%.Compared with control cervix or chronic convicitis,hTERC gene positive expression in CIN and SSC were different significantly(P<0.001). There was a significantly difference between CIN Ⅰand CIN Ⅰ/Ⅲ (P<0.05),also between CIN Ⅰ/Ⅲ and SSC. The expression rate of the hTERC gene was increased with the severity of the disease. The sensitivity、specificity、positive predicted value and negative predicted value of hTERC gene which detected in CINⅡ/Ⅲ were 86.67%、92.06%、83.87% and 93.53% respectively. Comparing with the control group,the rate of hTERC gene amplification was increased in CIN and SSC. We observed hyperdiploid cell in CIN and SSC. Conclusion:The increase of hTERC gene expression in the SCC and CIN suggests that hTERC gene serves as a screening test marker for helping to determine the progressive potential of individual lesions.
