胫骨平台双髁骨折内固定选择的生物力学比较研究

【摘要】 目的探讨胫骨平台双髁骨折采用三种不同形式内固定技术生物力学特性,为临床应用提供基础理论依据。方法采集新鲜冰冻成人膝关节标本12具,用实验应力分析方法和压敏片法,比较它们的强度、刚度、关节稳定性和关节接触特征量上的差异,以及它们之间的扭转力学性能。结果生物力学实验结果表明,胫骨平台双髁骨折采用三种不同形式内固定中以外侧支持钢板结合前内侧抗滑钢板力学性能最优,其次是外侧钢板结合内侧经皮拉力螺钉,而单侧锁定钢板为最差。结论胫骨平台双髁骨折采用外侧支持钢板结合前内侧抗滑钢板内固定最为可靠,外侧钢板结合内侧经皮拉力螺钉内固定其次,单侧锁定钢板内固定的稳定性较差。前两者在生物力学能上占有较大优势,固定后能较好的维持关节的静力稳定性和动力稳定性

【Abstract】 Objective To discuss biomechanical characteristics of bilateral tibial plateau fractures adoptions with 3 differerent internal fixation techniques, and offer basic theory for clinic. Methods With 12 fresh frozen autopsy adult knee joint specimens, using experimentation stress analytical method and pressure sensitive film method, the intensity, rigidity, joint stability, joint contiguity and torsion character within them were compared.Results The results of biological mechanical experimentation indicated: in the 3 differerent types of internal fixation techniques of bilateral tibial plateau fractures, the best group of mechanical capability was the outside supporting armor plate integrating former-inside counteractant sliding armor plate group (A group),secondly was the outside supporting armor plate integrating inside pulling bolt of the loose bone substance group (C group),while the last was the outside single locked armor plate group (B group). Conclusion In the bilateral tibial plateau fractures adoption 3 differerent shapes internal fixation techniques,the best is A group, sencondly is C group,the last is B group.there are great advantage of the two former in biologic mechanical characteristic, for they could maintain preferable arthral motionless and motive power stability after fixation.
