中药胆宁片对胆汁热力学平衡体系的影响
Influence of Danning tablet on the thermodynamic equilibrium system of bile
【摘要】 目的探讨胆宁片对胆汁热力学平衡体系的影响。方法选取2005年7月至2009年6月180例胆石症手术病例进行研究,病例分为胆固醇结石胆宁片组(n=67)、胆色素结石胆宁片组(n=25),并分别设立对照组(n=71;n=19);另外设正常人群组(n=20)。通过检测每份胆汁中结合胆红素、总胆红素、胆固醇、磷脂、胆盐浓度,计算出游离胆红素百分比(UCB%)、胆汁成石指数(LI)、胆汁成石趋势综合值(Z值),比较各组之间的差异。结果 在胆固醇结石组,胆宁片组与对照组相比较,其胆汁UCB%、LI、Z1值均明显降低(P值均<0.01);在胆色素结石组,胆宁片组与对照组比较,其UCB%、Z1值明显降低(P值均<0.01),但LI值差异不明显。结论胆宁片可影响胆汁热力学平衡体系的诸多因素,降低胆汁成石趋势。
【Abstract】 Objective To explore and discuss the influence of Danning tablet on the thermodynamic equilibrium system of bile. Methods One hundred and eighty patients with cholelithiasis undergone cholecystectomywere prospectively studied from July 2005 to June 2009 and divided into Danning tablet group with cholesterol gallstone (n=67), control group with cholesterol gallstone (n=71), Danning tablet group with pigmentstones (n=23), control group with pigment stones (n=19) and normal group (n=20). Bile total bilirubin, directbilirubin, cholesterol, phospholipid and total bile salt were measured for each sample, and the percentage ofunconjugated bilirubin (UCB%), the lithogenic index (LI) and Z1 value for estimating the lithogenic tendencywere calculated. Results Compared with the control group, in Danning tablet group, the UCB%, LI and theZ1 value decreased significantly (all P<0.01) for cholesterol gallstone group; the UCB% and Z1 value also decreased (P<0.01) for pigment gallstone group, but the difference of LI was not significant. Conclusion Dan-ning tablet can impact many factors of the bile thermodynamic equilibrium system, and thus may reduce thetendency of gallstone formation.
