【摘要】 目的探讨闭合性胰腺损伤的诊断和外科治疗。方法回顾性分析2003年～2008年我院收治的29例胰腺损伤的临床资料。结果 29例胰腺损伤中术中探查发现24例,术前明确诊断者仅5例。行裂口单纯缝合,被膜切开,血肿清除,胰周引流者18例;胰体尾切除5例;胰腺头端关闭,远端空肠Roux-en-Y吻合4例;胰十二指肠修复加旷置术2例。其中并发胰瘘5例,胰腺假性囊肿2例。治愈26例,死亡3例。结论剖腹探查术仍是胰腺损伤早期诊断最简单和最可靠的方法。不同的胰腺损伤类型选择不同的手术方式。重点是术前准确估计胰腺损伤的程度,关键是判断胰管损伤与否。更多还原

【Abstract】 Objective To explore the diagnosis and surgical treatment of blunt pancreatic injury.Method The clinical data of 29 cases with blunt pancreatic injury were analyzed retrospectively from 2003 to 2008.Resullts 24 cases were diagnosed through operation,preoperative correct diagnosis was made only in 5 cases.Suture and opening of the pancreatic capsule for drainage in 18 cases,body-tail of pancreatectomy in 5 cases,pancreatic gland head end closure,far-end jejunum Roux-en-Y tallies in 4 cases,pancreaticoduodenal restoration and diverticula in 2.5 cases of pancreatic fistulas,2 cases of pancreatic pseudocysts.26 cases are recovered,3 patients died.Conclusions Exploratory laparotomy is the most simple and effective method for early diagnosis of pancreatic injury.To select proper surgical procedures according to degree of pancreatic injury.The emphasis is to evaluate the degree of pancreatic injury accurately before operation,and the key is to determine the pancreatic duct injures or not.更多还原
