冠状动脉管壁高分辨力磁共振成像的初步研究

【摘要】 目的:探讨冠状动脉管壁的高分辨MR成像方法,评价其在显示冠状动脉细微解剖结构方面的应用价值。方法:选取20例健康志愿者在1.5T MR设备上行高分辨冠状动脉管壁断面成像,由两位高年资MRI医师对图像质量进行评分,并在评分较高的图像上测量血管内径及管壁厚度等数据。结果:18例志愿者均完成检查,共获得冠状动脉管壁横轴面图像97帧,其中右冠状动脉(RCA)59帧,平均得分2.15分;左冠状动脉(LCA)38帧,平均得分1.39分,RCA图像质量优于LCA。在RCA图像中质量评分≥2分的图像上测量血管内径及管壁厚度,RCA近段内径平均值为(3.55±0.51)mm,管壁厚度平均值为(1.11±0.23)mm。结论:冠状动脉管壁MR高分辨成像可以显示冠状动脉管壁细微解剖结构,并可定量测量和分析相关数据,在冠状动脉疾病的临床诊断及研究中具有较大的应用前景。

【Abstract】 Objective:To study the technique of high-resolution MR imaging of coronary artery(CA) wall,and to assess its value in demonstrating anatomic structure of CA wall in detail.Methods:20 healthy volunteers were selected to undergo high-resolution MR imaging of CA with a 1.5T MR system.All of the images obtained were scored by two senior radiologists in MRI major.The intra-luminal diameter of CA and the thickness of the wall were measured on cross-section images with higher scores.Results:18 volunteers had the study successfully completed and 97 cross section images of CA were obtained.Of all the images,59 were of right CA with the mean score as 2.15 and 38 were of left CA with the mean score as 1.39.The image quality of RCA was better than that of LCA.Measurement was performed on the cross section images of RCA with scores ≥2.The mean intra-luminal diameter of RCA was(3.55±0.51)mm and the mean wall thickness was(1.11±0.23)mm.Conclusion:The detailed anatomy structure and quantitative measurement of CA could be demonstrated on the high-resolution MR imaging,the data acquired could be used in clinical practice and research for CA disorders with good prospects.
