【摘要】 目的探讨青年与老年胃癌的临床、病理、治疗及预后等方面的特征。方法回顾性分析2000年1月至2005年12月收治的69例年龄<45岁青年胃癌的临床资料,并与同期年龄>65岁胃癌168例比较。结果与老年胃癌病人比较,青年组女性比例较高、有肿瘤家族史、临床表现缺乏特异性、首发症状至确诊时间长、BorrmannⅢ型及Ⅳ型为主、以低-未分化腺癌比例高、确诊时Ⅲ期及Ⅳ期居多、转移率高、多选择手术治疗、生存率低。结论青年人胃癌与老年人胃癌在性别构成、临床表现、好发部位、病理类型、预后诸方面有着明显的不同,应采取综合方法以提高诊断准确率。只要条件许可,应以手术为主。更多还原

【Abstract】 Objective To study the clinical,pathological,curative and prognostic characteristics of gastric cancer in young and old patients.MethodsRetrospective analysis was conducted on the clinical data from Jan.2000 to Dec.2005 recorded for 69 patients under the age of 45 and 168 patients over the age of 65 that suffered gastric cancer.ResultsCompared with mid-aged and old patients,the percentage of female patients in young patients’ group was higher,generally they got family history of cancer,the clinical symptoms were lack of specificity,the time elapsed between initial symptom and disease confirmation was long,Borrmann Ⅲ and Ⅳ cancers took the major parts,there was larger percentage of poorly differentiated adenocarcinoma and undifferentiated adenocarcinoma,stages Ⅲ-Ⅳ were more than others when diseases were confirmed,the metastasis rate was high,most patients chose surgical treatment,and survival rate was low.ConclusionThere was significant difference between young and old patients with gastric cancer in sex structure,clincial symptom,predilection sites,pathologic type and prognosis.Clinically,the comprehensive analysis should be used to promote the accuracy of diagnosis,and if condition permits,elective surgery should be considered the major treatment.更多还原
