大内淋巴管和内淋巴囊综合症的MRI诊断

【摘要】 目的探讨大内淋巴管和内淋巴囊综合症的MRI影像学特征,以提高对本病的认识。方法收集经MRI明确诊断的13例共26耳大内淋巴管和内淋巴囊综合症,所有病例均行MRI常规平扫和3D-CISS或3D-SPACE序列MIP三维重建,分析病变的形态及信号等影像学表现。结果大内淋巴管和内淋巴囊综合症表现为内淋巴管和内淋巴囊的不同程度扩张,其近端呈鸟嘴状与前庭相连。在横断位断面图像上,扩张的内淋巴管和内淋巴囊21耳呈典型的"汤勺状",另5耳呈饼状。在MIP三维重建图像上,扩张的内淋巴管和内淋巴囊在其冠状位上则呈典型的"树叶状"。病变呈长T1长T2信号,但在T2WI上扩张的内淋巴囊信号有不均匀减低。结论 MRI对大内淋巴管和内淋巴囊综合症具有较高的敏感性和特异性,在其诊断中起着重要的作用。

【Abstract】 Objective To investigate MR imaging features of large endolymphatic duct and sac syndrome for improving understanding of the disease. Methods MRI imaging findings such as morphology and signal of large endolymphatic duct and sac syndrome (n=13 cases, 26 ears) were analyzed retrospectively. All of 13 cases underwent routine MRI scanning and 3D-CISS or 3D-SPACE sequence MIP three-dimensional reconstruction. Results Large endolymphatic duct and sac syndrome manifested as dilation of endolymphatic duct and sac in different degrees, and its beak-shaped proximal end was connected with the vestibule. In the MR axial cross-section images, the dilated endolymphatic duct and sac displayed a typical "spoon-shaped"(n=21 ears), or"cakeshaped"(n=5 ears). In the MIP 3-D reconstruction images, all of them showed a typical " leaf-shaped " in themself coronal. The signal intensity of the dilated endolymphatic duct and sac were low on T1WI and high on T2WI, but non-homogeneous decreased signal intensity were found on T2WI in the dilated endolymphatic sac. Conclusion MRI has a higher sensitivity and specificity in large endolymphatic duct and endolymphatic sac syndrome, and plays an important role in making diagnosis for it.
