银杏叶提取物对慢性缺血性脑病的治疗作用研究

【摘要】 目的探讨银杏叶提取物(GBE)对慢性缺血性脑病的治疗作用。方法选择2005年1月至2008年12月该院神经内科收治的患者中确诊为慢性脑供血不足(CCCI)患者109例,分为治疗组和对照组。治疗组使用GBE治疗,对照组使用香丹注射液治疗。观察患者0、5、10 d的疗效,经颅多普勒超声(TCD)测定椎基动脉收缩期峰值流速(Vs)变化和血流变学参数变化。结果 GBE治疗5 d后,治疗组治愈率、显效率、有效率开始提高;全血黏度、血浆黏度、红细胞聚集度和红细胞比容等血流变学参数开始改善,与对照组比较,差异有统计学意义(P<0.05)。治疗10 d后,治疗组治愈率、显效率改善更明显,与对照组及治疗5 d比较,差异有统计学意义(P<0.05);椎基动脉Vs变化在左侧椎动脉(LVA)、右侧椎动脉(RVA)和基底动脉(BA)明显改善,与对照组比较,差异有统计学意义(P<0.05);全血黏度、血浆黏度、红细胞聚集度和红细胞比容等血流变学参数均改善,与对照组及治疗5 d比较,差异有统计学意义(P<0.05)。结论 GBE药理学作用广泛,能较全面覆盖CCCI的众多病理生理环节,在临床使用时治愈率高,改善脑血流和血流动力学功能显著。

【Abstract】 Objective To approach on the clinical effects of the treatment of chronic cerebral circulation insufficiency(CCCI) with Ginkgo Biloba Extract(GBE).Methods One hundred and nine patients with cerebral circulation insufficiency were randomly divided into two groups in the neurologic department:therapy group(T group) and control group(N group).The therapy group was treatmented with GBE,the control group was treated with Xiangdan Injection.We observed the effects after 0,5,10 d therapy.Results The changes of Vs and hemodynamic parameters of vertebrobasilar were determined with transcranial Doppler.After 5 d GBE treatment and the cure rate,markedly effective rate of the T group were markedly better than those of the N group(P<0.05),while there was statistical significance in hemorheology improvement(P<0.05),such as blood viscosity,plasma viscosity,erythrocyte aggregation and hematocrit.After 10 d GBE treatment,the above parameters were significantly improved.The changes of LVA,RVA and BA of vertebrobasilar were also significantly improved(P<0.05).Conclusion There are wide range of pharmacological effect of GBE,it can cover many aspects of pathophysiological links of CCCI.GBE has the functions of improving clinical symptoms,cerebral blood flow and hemodynamics of CCCI.
