肽受体放射性核素治疗进展期胰腺神经内分泌肿瘤

【摘要】 目的进展期胰腺神经内分泌肿瘤缺乏有效的治疗方法,肽受体放射性核素治疗是针对不能手术和转移病灶的一种新方法。本文对90Y-生长抑素类似物([90Y-(DOTA)0,Tyr3]octreotate,90Y-DOTATATE)的疗效及副作用进行了观察。方法 6例进展期胰腺神经内分泌肿瘤患者,生长抑素受体显像诊断为生长抑素受体阳性肿瘤。按体表面积计算,90Y-DOTATATE累积剂量为7.4MBq/m2,分成3~5次治疗,间隔时间为6~9周。90Y-DOTATATE在30分钟内静点完毕,用药前后输注赖氨酸。每次单程治疗后以美国国立癌症研究院分级标准评价不良反应。总疗程结束后8周以WHO标准评价疗效。结果轻微缓解2例,病情稳定2例,病情进展2例。1例出现轻度肾功能损害,1例患者出现白细胞下降,3例出现恶心。结论肽受体放射性核素治疗对转移和不能手术的胰腺神经内分泌肿瘤有一定疗效。

【Abstract】 Objective To evaluate the efficacy and side effects of90Y-DOTATATE([90Y-(DOTA)0,Tyr3]octreotate) in treating metastasized or inoperable neuroendocine tumors.Methods Six patients with progressive pancreatic neuroendocrine tumors were included.All of them were confirmed to be SSTR2-positive tumors by somatostatin receptor scintigraphy.The cumulative dose of 90Y-DOTATATE was 7.4MBq/m2.Each patient received three to five cycles of treatment and 90Y-DOTATATE was administered intravenously within thirty minutes.The interval between these two cycles was 6-9 weeks.Lysine was injected before and after the administration of 90Y-DOTATATE.After each treatment cycle, the side effects were assessed according to National Cancer Institute Grading Criteria(NCIGC).The efficacy was evaluated by the WHO criteria 8 weeks after the whole treatment.Results A minor response was found in 2 patients,stable disease in 2 patients,and tumor progression in 2 patients.One patients developed renal toxicity,1 patients developed leukocytopenia,and 3 patients showed nausea.Conclusions 90Y-DOTATATE provids a promising new tool for the management of patients with advanced pancreatic neuroendocrine tumors.
