【摘要】 目的通过测定不同严重程度的骨关节炎患者的血清IL-4和sIL-4R水平,对IL-4和sIL-4R水平与骨关节炎严重程度的相关性进行分析研究。方法选取临床收治的膝关节骨关节炎病例共42例作研究,选取12名性别比例、体质量指数相当的正常人作对照组。用WOMAC骨关节炎指数对骨关节炎严重程度进行临床评估。用K&L评级法对骨关节炎严重程度进行影像学评估。用ELISA法检测研究对象血清IL-4和sIL-4R水平。结果骨关节炎组和正常对照组血清IL-4浓度在0~1.582pg/ml,差异无统计学意义。骨关节炎组的血清sIL-4R水平显著高于正常对照组(Z=-4.236,P=0.00),差异有统计学意义。骨关节炎组的WOMAC指数与血清sIL-4R水平存在相关关系,相关系数为0.366(P=0.017)。正常对照组与骨关节炎各K&L等级组的血清sIL-4R水平比较差异有统计学意义(χ2=29.219,P=0.00),各组的sIL-4R水平均数呈递增趋势。结论骨关节炎患者血清IL-4水平无明显升高,骨关节炎患者sIL-4R水平显著高于正常对照组,而且sIL-4R水平与OA的严重程度具有相关性,血清sIL-4R水平可作为OA的诊断及严重程度判断的参考指标。更多还原

【Abstract】 Objective To study correlation between serum IL-4 or sIL-4R and severity of osteoarthritis(OA).Methods 42 OA cases admitted from Jan 2008 to June 2009 were enrolled in the study,including 12 males and 30 females,with average age of 58 and average history of 12 years.12 healthy individuals with matched age,gender and body-mass index were enrolled as normal control(NC).Clinical severity of OA was evaluated with WOMAC index.Radiological severity of OA was evaluated with K&L score.Serum IL-4 and sIL-4R were measured with enzyme-linked immunosorbent assay(ELISA).Results Serum IL-4 level of both OA and NC groups was 0-1.582 pg/ml,without statistical differences.Serum sIL-4R level of OA group was significantly higher than that of NC group(Z=-4.236,P=0.00),with statistical differences.There was correlation between WOMAC index and serum sIL-4R level with relation coefficient of 0.336(P=0.017).Comparison of serum sIL-4R level among NC group and K&L groups of OA showed that there were statistical differences among groups(χ2=29.219,P=0.00),with increasing tendency.Conclusion There is no significant increase of serum IL-4 level in OA group.Serum sIL-4R level of OA group was significantly higher than that of NC group,and there is correlation between serum sIL-4R and severity of OA.Serum sIL-4R can be used as a reference biomarker of severity of OA.更多还原
