【摘要】 目的探讨胎儿心室内强回声点(EIF)在高风险组与低风险组中及孤立出现和联合其他软标志出现时的临床意义。方法3344例无明显结构异常胎儿分成高、低风险组,发现EIF的胎儿依据风险高低及其是否孤立出现,分别采取分娩时取脐血检查及妊娠期间行羊水或脐血穿刺进行染色体核型分析的方法,计算其EIF及染色体异常的检出率。结果高风险胎儿EIF检出率高于低风险胎儿,有EIF的胎儿染色体异常的检出率高于低风险组。高风险组22例EIF,19例EIF孤立存在的病例中有2例染色体异常,3例合并其他超声软标志的病例中有2例染色体异常;低风险组50例EIF,46例EIF孤立存在的病例中无一染色体异常,4例合并其他超声软标志的病例中有1例染色体异常。结论低风险组中孤立的EIF临床意义不大;高风险组中孤立的EIF有临床意义,须结合多方面临床资料,谨慎对待。如EIF合并多项超声软标志,则不论是高风险组还是低风险组胎儿均与染色体异常密切相关,建议作染色体核型分析。更多还原

【Abstract】 Objective To explore the meaning of fetus’s ventricle echo (EIF) in the high-risk and the low-risk focus when appeared isolatedly or combined with other floppy appearances. Methods A total of 3344 cases that had no obvious unwonted configuration fetus were devided into high-risk group and low-risk group,and were analyzed according to the risk and the EIF appearance,partly using the pursuing method and the chromosome karyotype analyzing to account the detection rate of the EIF and the unwonted chromosome. Results The detection rate of EIF in the fetus of high-risk group was higher than that of the low-risk group,and same in the unwonted chromosome. In 22 cases with EIF of the high-risk group,19 cases with EIF were isolated,including 2 cases of unwonted chromosome,and 2 cases were diagnosed unwonted chromosome in the left 3 cases with EIF combined other ultrasound floppy sign. In 50 cases with EIF of the low-risk group,46 cases with EIF were isolated and had no case unwonted chromosome,and 1 case was diagnosed unwonted chromosome in the left 4 cases with EIF combined other ultrasound floppy sign. Conclusion The isolated EIF in the low-risk group has no significant clinical meaning,but in the high-risk group does,and it must be analyzed based on many-sided clinical data and treated cautiously. In cases of EIF combined with multinomial ultrasound floppy sign,whether in the high-risk or the low-risk group,the fetus may have many affinities with the unwonted chromosome,and analysis of the chromosome karyotype is suggested.更多还原
