脑组织内甘露醇含量与脑出血后脑水肿的关系研究

【摘要】 目的探讨大鼠脑出血连续使用甘露醇后脑组织内甘露醇含量与脑水肿的关系。方法采用立体定向仪注射胶原酶制作大鼠右侧尾状核脑出血模型,再按1.0 g/kg体重每6 h静注20%甘露醇,分别于出血后6、12、24、48 h取出血灶周边脑组织标本,通过测定脑组织中甘露醇含量、水等指标,评价连续使用甘露醇对脑出血后脑水肿的影响。结果 (1)脑出血+静注甘露醇组中,出血侧各亚组(DT组)甘露醇含量明显高于同时间点健侧(NT组)(P<0.001),且随处理时间延长而升高;出血侧(DT组)和健侧(NT组)甘露醇含量均于24 h后明显升高。(2)脑出血+静注甘露醇组中,出血侧(DT组)24、48 h脑组织含水量明显高于同时间点脑出血对照组出血侧(DC组)脑组织含水量(P<0.005)。结论连续多次应用(2次以上)甘露醇可使其在脑组织中积聚,其程度可随使用次数的增加而加重,且时间持续到24 h后反而使出血侧脑水肿加重。甘露醇1 g/kg,每6 h静注治疗大鼠脑出血后脑水肿,对健康脑组织能发挥脱水作用。

【Abstract】 Objective To explore the effect of mannitol in brain tissue on brain edema and the relationship between them after intracerebral hemorrhage(ICH) in rats.Methods ICH model of rats were established firstly.All of these forty health adult rats were randomly divided into two groups,with twenty in each one.The treatment group suffered injection of mannital,and the control group without injection.Each group was further divided into four teams,five in each one.All rats were killed at 6,12,24,48 hour for measuring brain water contents and mannitol contents in injurious area and health area of brain tissue.Results(1)In group DT,brain mannitol content at 24,48 hour after ICH was significantly higher than the other two 6,12 team(P<0.01);(2) In group DT,brain water content was higher than group DC(P<0.005).Conclusion (1)After multiple mannitol injections,the mannitol can accumulated in brain tissue.(2) The effect of mannitol in brain edema decreased and even exacerbated with multiple mannitol injections.
