慢性鼻窦炎筛泡黏膜上细菌生物膜形态观察

【摘要】 目的寻找慢性鼻窦炎粘膜上细菌生物膜存在的证据。方法取3例需内镜下行鼻窦开放术患者的筛泡粘膜,约3.0mm×3.0mm大小,4%中性甲醛固定。四氧化锇固定,二氧化碳气体中临界干燥,表面镀膜喷金,电子显微镜扫描。图像保存为图片并打印。结果细菌生物膜被发现,其特征结构如水道、3-D结构和矩阵嵌入球体和椭球体内。3例慢性鼻窦炎患者筛泡粘膜上均发现细菌生物膜的存在。结论通过鼻窦手术获得慢性鼻窦炎患者的标本上检测到细菌生物膜,这是国内首次发现慢性鼻窦炎粘膜上存在细菌生物膜。

【Abstract】 【Objective】Biofilms recently have been associated with numerous chronic infectious processes. Many researchers considered the biofilms are also associated with chronic rhinosinusitis. To look for more evidences, we detected whether bacterial biofilms exist in chronic rhinosinusitis. 【Methods】The tissue samples from 3 patients undergoing endoscopic sinus surgery (ESS) for CRS were obtained. All surgical procedures were performed by the senior author. Tissue samples were obtained from the ethmoid bulla. Sample size varied about 3.0 mm×3.0 mm. After directly swabbing the specimens in a sterile container, specimens were fixed with 4% Formalin, then dehydrated with successive immersions in increasing concentration dilutions of ethanol. The specimens were allowed to air dry and then fixed onto aluminum stubs by using colloidal carbon. The sample surface was sputter coated with a gold and palladium layer. The specimens were examined with scanning electron microscope. Images were viewed in the SEM and then exposed onto negative film, scanned, and printed. 【Result】Biofilm findings such as water channels, 3-D structure, and matrix-embedding spherical or elliptical bodies within the size range of bacterial organisms. All tissue samples from 3 patients were found that the bacterial biofilms exist in the mucosal surface with chronic rhinosinusitis. 【Conclusion】Biofilms were identified in samples of tissue obtained during sinus surgery in patients with CRS. In china, this appears to be the first evidence of biofilms in CRS.
