干化学法与湿化学法测定血氨及影响因素的分析

【摘要】 目的比较干化学法与湿化学法测定血氨水平及分析影响血氨测定的因素。方法分别用干化学法和湿化学法立即测定35例患者的无氨管和普通管血氨值,并对10例样本在4℃和25℃时进行0、0.5、1、2和4h的动态检测,并进行统计分析。结果干化学法测定血氨在(16~486)μmol/L内线性良好,湿化学法测定血氨在(15~473)μmol/L内线性良好;批内重复性较好。干化学法测定血氨水平高于湿化学法,普通管测定的血氨值高于无氨管,但立即检测差异无显著性(P>0.05)。随着标本放置时间的延长,血氨值会不断的升高,温度升高变化更明显,差异有显著性(P<0.05)。结论干化学法和湿化学方法测定血氨线性及批内重复性良好;临床上的血氨标本应采用无氨管,采集标本后应迅速送检立即检测,放置时间不得超出1h。

【Abstract】 【Objective】To compare the dry and wet chemical method of chemical analysis of blood ammonia levels and the factors that affect the blood ammonia determination. 【Methods】The values of blood ammonia of 35 cases of no ammonia tubes and general tubes are immediate tested by dry chemical and wet chemical method, respectively. A sample of 10 cases lying in 4 ℃ and 25 ℃ at 0 h, 0.5 h, 1 h, 2 h and 4 h of the dynamic testing, and statistical analysis. 【Results】 The results of dry chemical method in the determination of plasma ammonia from 16 μmol/L to 486 μmol/L with well linear, wet-chemical determination of plasma ammonia from 15 μmol/L to 473 μmol/ L with well linear, and approved with better repeatability. The blood ammonia levels tested by dry chemical were higher than the wet chemical method. The blood ammonia value in general tubes were higher than in the no ammonia tubes, but the immediate detection of the difference was not significant (P >0.05). With the extension of storage time samples, blood ammonia values will continue to rise, more obvious changes in temperature, the difference is statistically significant (P <0.05). 【Conclusions】The blood ammonia levels tested by dry chemical and wet chemical methods with well linear and approved good repeatability. Clinical specimens should be used with no ammonia tubes, collecting specimens for detection immediately after it was taken out of body, storage time should not exceed 1h.
