【摘要】 目的探讨药物特利加压素对肝大部切除术后大鼠小体积残肝的保护性治疗及其可能机制。方法将70%肝切除大鼠分为对照组和实验组(即特利加压素组,Terlipressin),门脉血流恢复灌注后0.5、2、4、6和24h5个时间点取材、送检,主要进行肝功能检测及肝组织病理组织学检查;免疫组织化学方法检测肝组织中A20、iNOS、ET-1的表达情况。结果大鼠各时点肝功能指标明显上升,以6h和24h点最为明显,实验组低于对照组(P<0.05);光镜下各时点实验组肝组织损伤形态学改变都较对照组轻微;各时间点实验组中的A20表达上调较对照组明显,以再灌注后2h最为显著;而iNOS和ET-1的表达上调在对照组中更明显。结论特利加压素对肝大部切除术后大鼠小体积残肝有保护作用,能改善小体积残肝再灌注后早期炎性反应,减轻肝细胞凋亡,维持残肝微循环稳定,使小体积残肝功能损伤有所降低。更多还原

【Abstract】 【Objective】To investigate Terlipressin’s protective strategy and mechanism on the remnant livers after major hepatectomy.【Methods】Rats were divided into two groups after 70% hepatectomy:Control group,with normal salinel Terlipression group,with Terlipressin being administrated 10 mins preoperation.At different time points after portal reperfusion,the serum liver function were measured,the liver tissues were harvested and studied with pathological observation using HE stain and the expressions of A20,iNOS,ET-1 were detected by immunohis-tochemistry.【Results】At 6 h and 24 h time points,the levels of aminotransferase and total bilirubin were signifi-cantly decreased in the Terlipressin group when compared with the Control group(P <0.05),though both of them increased saliently.A20 was partly expressed at 0.5 hour timepoint and reached peak at 2 hours timepoint,and it expressed more markedly in the Terlipressin group rats than the control group at the same time points(P <0.05),whereas iNOS and ET-1 expressed markedly increased at the relative time points in the control group(P <0.05).【Conclusions】 Terlipressin can protect the function of remnant liver at the early phase after portal reperfusion by ameliorating the overwhelming inflammatory response,attenuating hepatocyte apoptosis and helping stabilize microculatory homeostasis.更多还原
