分水岭脑梗死部位与颈、脑大动脉病变的关系
Relationships between locations with Cerebral Watershed Infarction(CWI) and the distribution of Stenotic or Occluded Large Arteries in Neck and Brain
【摘要】 目的探讨脑分水岭梗死(CWI)部位与颈、脑大动脉狭窄及闭塞的关系。方法对52例常规头颅CT和MRI检查证实为分水岭梗死脑梗死患者行MRA检查颈、脑大动脉病变情况。结果52例患者大动脉狭窄、闭塞总发生率92.3%(48/52),其中颈内动脉及大脑中动脉占84.7%。皮质前型病灶颈内动脉病变59.3%(16/27),大脑中动脉病变40.7%(11/27),皮质下型病灶颈内动脉病变40%(12/30),大脑中动脉病变60%(18/30)。皮质后型病灶颈内及大脑中动脉病变87.5%(14/16),大脑后动脉病变37.5%(6/16),基底动脉病变18.75%(3/16)。结论颈内动脉及大脑中动脉狭窄或闭塞是脑分水岭梗死的主要病理基础。
【Abstract】 Objective To explore the relationships between locations with CWI and the distribution of stenotic or occluded large arteries in neck and brain.Methods 52 patients with CWI demonstrated by computer tomography(CT) and/or magnetic resonance imaging(MRI) are examined by MRA(magnetic resonance angiography) to collect data concerned with large arteries in neck or brain.Results 48 of 52 patients with CWI have stenotic or occluded large arteries in neck and brain(total stenotic or occluded proportion 92.3%);the proportion of internal carotid arteries(ICAs) or middle cerebral arteries(MCAs) involved in is 84.7%.The distribution of ICA and MCA lesions in precortical type is 59.31%(16/27) and 40.7%(11/27),respectively;The distribution of ICA and MCA lesions in subcortical type is 40%(12/30) and 60%(18/30),respectively;The distribution of ICA plus MCA,posterior cerebral artery as well as basilar artery lesions in postcortical type is 87.5%(14/16),37.5%(6/16) and 18.75%(3/16),respectively.Conclusion Stenotic or occluded ICAs and MCAs are a main pathological factor for CWI.
