主动免疫治疗中迟发型超敏反应的初步研究
Analysis of delayed-type hypersensitivity (DTH) in active specific immunotherapy (ASI) for patient with advanced malignant tumor
【摘要】 目的探讨新城鸡瘟病毒修饰的自体肿瘤疫苗(NDV-ATV)主动免疫治疗(ASI)前后机体的细胞免疫功能变化。方法对32例术后的进展期恶性肿瘤患者,在NDV-ATV接种前及第4次接种后一周,检测机体对肿瘤抗原的迟发型超敏反应(DTH)反应,每个患者均以PPD作为对照。接种前后,在12例患者DTH反应部位取皮肤活检,Envision二步法免疫组化染色,检测致敏的CD8+T细胞、CD68+巨噬细胞数。抽取外周血检测接种前后的CD4+T细胞及CD8+T细胞的变化。结果肿瘤抗原的DTH值,主动免疫治疗后明显大于接种前;免疫治疗后外周血CD4+T细胞及CD8+T细胞比例明显升高;免疫治疗后DTH部位真皮组织内CD8+T细胞、CD68+巨噬细胞数目明显增多。结论NDV-ATV能够改善机体的细胞免疫功能。
【Abstract】 Objective To evaluate the immunological function of patients with advanced malignant tumor after active specific immunotherapy using new castle disease virus-modified autologous tumor vaccine(NDV-ATV).Methods Thirty-two post operative patients with advanced malignant tumor were enrolled in this study.before ASI and after the fourth innoculation,we injected sepecfic tumor antigen and purified protein derivative(PPD)in homologous hypoderma as a control to detect the value of delayed-type hypersensitivity(DTH)and the erythema and induration value initiated by PPD.Among the 12 patients,we got the skin tissue of DTH site to make Envision immuno-histochemistry staining so as to count the number of sensitized CD8+Tcells and CD68+macrophages before ASI and after the fourth inoculation.Intracellular cytokine of CD4+Tcell and CD8+Tcell in PBMNC were detected by flow cytometry before and after the inoculation.Results DTH numerical value increased after ASI.After ASI,The serum level ofCD4+T cell and CD8+Tcell was elevated obviously.The number of sensitized CD8+Tcells and CD68+macrophages in the DTH site was enlarged.Conclusion Inoculation with NDV-ATV can improve the cell-mediated immunological function of tumor patients.
