【摘要】 目的分析团队管理群组模式在社区糖尿病管理中干预的效果,为推广所创建的新型社区糖尿病管理模式提供依据。方法按照以社区为基础的随机对照试验研究设计,将208名自愿参加的糖尿病病人随机分为干预组和对照组。比较两组糖尿病病人在项目实施12个月后在自我管理行为、自我效能、健康状况、血糖和血压控制及卫生服务利用等方面的变化。结果干预组干预后认知性症状管理技能实践评分比对照组平均增加了0.34分(P<0.05);糖尿病总体管理自我效能评分比对照组高0.71分(P<0.05);干预组干预后收缩压比对照组降低3.70 mm Hg(P<0.05)。结论团队管理群组模式能帮助糖尿病病人改善自我管理行为、部分健康状况及降低血压水平。更多还原

【Abstract】 Objective To analyze the 1-year intervention effectiveness of Group Visits model in the community-based diabetes management,and to find scientific evidence to support the expansion of the newly established Group Visits model in our district.Methods The study employed a community-based randomized controlled trial design.208 community-dwelling patients with diabetes were randomly assigned into intervention group (n=119) and control group (n=98).Nonparametric test (Mann-Whitney Test)was used to compare the 12-month difference in self-management behavior,self-efficacy to manage diabetes, health status,Hb1c and blood pressure levels as well as health service utilization between the two groups.Results After 12- month intervention,compared with controls,patients in intervention group had significant improvements in practice of cognitive symptom management (improved 0.34 score,P=0.04),self-efficacy to manage diabetes in general (improved 0.71 score,P=0.02). They also had significant improvements in systolic blood pressure (improved about 3.70 mm Hg,P<0.05).Conclusion The Group Visits model in the community-based diabetes management improved participants’ self-management behavior,self-efficacy,and blood pressure control,but did not show a significant effect on the glycemic control.更多还原
