不同配比头孢呋辛钠/舒巴坦钠的体外药效学研究

【摘要】 目的评价头孢呋辛钠/舒巴坦钠(cefuroxime/sulbactam,CXM/SB)的体外抗菌活性。方法采用平皿二倍稀释法测定CXM/SB对1436株临床分离菌的MIC分布、试管二倍稀释测定最低杀菌浓度、活菌落计数法测定杀菌曲线。结果CXM/SB1∶1和CXM/SB 2∶1对1436株临床分离菌具有广谱抗菌作用,抗菌活性与哌拉西林/三唑巴坦、头孢哌酮/舒巴坦相近,强于CXM/SB 3∶1、4∶1、8∶1。CXM/SB 1:1和CXM/SB 2∶1对1109株产β-内酰胺酶菌株的MIC90值比CXM低4~32倍,对产ESBLs大肠埃希菌、肺炎克雷伯菌的MIC90值比CXM低16和8倍。不同pH值、细菌接种量、血清浓度对CXM/SB 2∶1抗金葡菌、大肠埃希菌、肺炎克雷伯菌的活性无明显影响。结论SB增强了CXM的抗菌活性,CXM/SB 1∶1和CXM/SB 2∶1具有较强和相近的广谱抗菌活性。

【Abstract】 Objective To investigate the in vitro antibacterial activities of cefuroxime/sulbactam(CXM/SB)in different compositions.Methods The in vitro antibacterial activity of CXM/SB against clinical isolated bacteria was determined by agar dilution method.The minimal bactericidal concentrations were determined by two-fold broth dilution method.The bacterial time-killing curves were acquired by viable bacteria count.Results The CXM/SB 1∶1 and 2∶1 displayed potent and broad-spectrum antibacterial activities against 1436 stains of clinical isolated bacteria,which were similar to piperacillin/tazobactam and cefoperazone/sulbactam and better than CXM/SB 3∶1,4∶1 and 8∶1.MIC90 of CXM/SB 1∶1 and 2∶1 was lower than CXM by 4~32 folds against 1109 stains of β-lactamases producing bacteria,and was lower by 16 folds and 8 folds against ESBLs-producing E.coli and K.pneumoniae respectively.The inoculum size,pH and serum concentration have no apparent effect on the inhibitory activity of CXM/SB 2∶1 against S.aureus,E.coli and K.pneumoniae.Conclusion SB enhanced antibacterial activity of CXM.CXM/SB 1∶1 and 2∶1 have the similar pharmacodynamic functions with potent and broad-spectrum antibacterial activity.
