鲍曼不动杆菌主动外排系统AdeABC、AdeIJK与多重耐药相关性

【摘要】 了解本地区鲍曼不动杆菌主动外排系统的分布并探讨其在介导细菌多重耐药中的作用。方法琼脂二倍稀释法检测20种抗菌药物对鲍曼不动杆菌临床分离株的MIC;PCR扩增AdeABC-AdeRS、AdeUK两种RND主动外排系统的结构和调节基因。结果112株鲍曼不动杆菌临床分离株对亚胺培南/西司他丁和美罗培南耐药率分别为0.9%和1.8%,对大多数抗菌药物的耐药率均大于60%,但对多黏菌素B和米诺环素均敏感。75.9%的菌株同时检出adeABC-adeRS和adeIJK主动外排基因,6.3%仅检出adeIJK。同时检出adeABC-adeRS和adeIJK的菌株耐药率较高。结论本地区鲍曼不动杆菌临床分离株多重耐药情况严重。adeABC和adeIJK在鲍曼不动杆菌临床分离株有较高检出率。AdeABC主动外排系统在鲍曼不动杆菌多重耐药性的形成中起重要作用。

【Abstract】 Objective Analyze the distribution and status of active efflux systems in clinical isolates ofAcinetobacter baumannii. Methods The minimal inhibitory concentrations (MICs) of 20 antibacterials against 112Acinetobacter baumannii clinical isolates were determined by agar dilution method according to CLSI guidelines(2006). The chromosomal DNA was amplified by PCR using primers of a deABC-adeRS and adelJK. Results Allisolates were fully susceptible to polymyxin B and minocycline. The resistance rates of the isolates to imipenem/cilas-tatin, meropenem, and cefoperazone-sulbactam were 0.9%, 1.8% and 37.2%, respectively, and more than 60 %of the isolates showed resistance to the other antibacterials tested. 75.9% (85/112) of the isolates detected werepositive for adeRS-adeABC and adelJK, which displayed a high level of multidrug resistance. 6. 3% of the isolateswere positive for adelJK only. Conclusions Most isolates were highly resistant to the antibacterial agents. The highfrequency of the coexistence of adeABC and adelJK efflux genes was found in A. baumannii clinical isolates. Activeefflux system AdeABC played an important role in multidrug resistance of Acinetobacter baumannii.
