耐甲氧西林金黄色葡萄球菌分子流行病学研究

【摘要】 目的研究湖南省4所综合医院分离耐甲氧西林金黄色葡萄球菌(MRSA)分子流行病学特点,为临床预防、监测MRSA的流行,并制定合理的防治方案提供确实可靠的依据。方法采用质粒图谱分析4所医院MRSA是否存在质粒流行,并分析其型别;采用RAPD分析对住院患者、环境、医务人员分离的MRSA进行同源性分析。结果对89株MRSA进行质粒图谱分析,有7株未检测到质粒,82株携带有质粒,可分为17型,4所医院MRSA均存在1~2种主要质粒的流行;经RAPD分型89株MRSA可分为A、B、C、D、E 5型,其中A型85株占所有菌株的95.50%,B型1株、C型1株、D型1株、E型1株。结论质粒图谱分析及RAPD分型方法可很好用于MRSA分子流行病学分析,医院MRSA临床株与环境株及湖南省4所综合医院之间MRSA菌株具有同源性,呈暴发流行。

【Abstract】 OBJECTIVE To study the characteristic of molecular epidemiology of MRSA isolated from 4 hospitals in Hunan Province,and provide reliable evidence for preventing and monitoring the spread of MRSA.METHODS The epidemic strains were analyzed by plasmid profile.Homology of the MRSA strain isolated from hospitalized patients,hospital environment and medical staff was analyzed by random amplification of polymorphic DNA(RAPD).RESULTS Seven strains were not detected out plasmid among 89 strains MRSA.Eighty-two strains were typed into 17 distinct profiles by plasmid profile.There were one or two kinds of epidemic plasmid of MRSA in the four hospitals.A total of 5 RAPD types(A,B,C,D and E) were categorized from the 89 strains,and 95.50% strains were type A.CONCLUSIONS The plasmid profile and RAPD assay are good methods for MRSA molecular epidemiology analysis.The MRSA strains are homologous,and have been causing an outbreak in 4 hospitals.
