肺栓塞患者心电图ST段压低与严重临床事件关系的临床研究
Prognostic Value of Electrocardiographic ST Segment Depression in Patients With Pulmonary Embolism
【摘要】 目的:通过观察44例肺栓塞患者心电图ST段的变化,探讨肺栓塞患者心电图ST段压低与心肌损伤以及严重临床事件发生的关系。方法:对44例患者进行前瞻性研究。根据心电图ST段变化,将患者分为两组:ST段压低组(23例)和非ST段压低组(21例)。同时均于24 h内相隔8~12 h两次采血测定血清肌钙蛋白I(TnI),两次都出现阳性结果,即为TnI阳性。观察住院期间两组患者的临床表现,对严重临床事件及死亡情况进行统计。结果:44例肺栓塞患者心电图ST段压低见于Ⅱ、Ⅲ、aVF、V1~4导联。两组中共有12例TnI阳性,其中ST段压低组11例,非ST段压低组1例,两组相比P=0.0014;共有13例次发生严重临床事件,其中ST段压低组11例次,非ST段压低组2例次,两组相比P=0.0054;死亡病例6例,ST段压低组5例,非ST段压低组1例,两组相比P=0.0209。结论:肺栓塞患者心电图ST段压低提示心肌受损及严重临床事件的发生率较高。
【Abstract】 Objective:To evaluate the correlation between electrocardiographic(ECG)ST segment depression with myocardial injury and the risk of complicated clinical incidence in patients with pulmonary embolism(PE). Methods:A total of 44 PE patients were divided into two groups:ECG ST segment depression group(n=23)and None ECG ST segment depression group(n=21).All patients’serum troponin I(TnI)levels were examined and repeated at 8-12 hours of interval within 24 hours,the positive result was defined as TnI≥0.07 ug/L,if it repeated twice,the patient was diagnosed as TnI positive.Other complicated incidence of adverse clinical events and mortality were analyzed and compared in both groups up to 14 days of admission. Results:In all 44 PE patients,ECG ST segment depression was observed inⅡ,Ⅲ,aVF and V1~V4 lead.There were 12 cases showed TnI positive,of which 11 from ECG ST segment depression group,and 1 from None ECG ST segment depression group (P=0.0014).There were 13 cases presented the complicated incidence of adverse clinical events,of which 11 from ECG ST segment depression group and 2 from None ECG ST segment depression group respectively(P=0.0054).6 patients died,of which 5 from ECG ST segment depression group and 1 from None ECG ST segment depression group respectively(P=0.0209). Conclusion:ECG ST segment depression in PE patients suggested myocardial injury with the elevated risk of complicated incidence of adverse clinical events.
