坎地沙坦与苯那普利联用治疗高血压伴微量白蛋白尿患者的临床观察

【摘要】 目的:观察坎地沙坦和苯那普利联用与应用其中任一单药治疗高血压伴微量白蛋白尿患者疗效的差异。方法:95例临床确诊的高血压伴微量白蛋白尿的患者随机分成3组,服用苯那普利10~20 mg/d为苯那普利组(n=31);服用坎地沙坦4~8 mg/d为坎地沙坦组(n=32);服用苯那普利10 mg/d+坎地沙坦4 mg/d为苯那普利+坎地沙坦组(n=32)。疗程8周,治疗前后检测尿微量白蛋白/肌酐(A/C)、收缩压、舒张压、血浆内皮素-1(ET-1)、血浆一氧化氮(NO)、血肌酐、血钾的变化。结果:3组患者在治疗8周后A/C值、血压较治疗前均有明显下降(P<0.05),血浆NO增加、血浆ET-1水平均降低(P<0.05),治疗8周后苯那普利+坎地沙坦组A/C、ET-1水平较苯那普利组或坎地沙坦组降低更明显(P<0.05),而NO则增加更明显(P<0.05),差异均有统计学意义。3组治疗后血肌酐、血钾差异均无统计学意义(P>0.05)。结论:苯那普利和坎地沙坦联合治疗比单药治疗更能减少高血压尿微量白蛋白,这一有益作用可能与改善血管内皮功能作用有关。

【Abstract】 Objective:To compare the effect between benazepril and candesartan combination and single medication in either one of them for treatment of hypertension-combined with microalbuminuria(MCA). Methods:A total of 95 patients were divided into three groups.Benazepril group(n=31),patients were treated with Benazepril 10~20 mg/day;Candesartan group(n=32),patients were treated with Candesartan 4~8 mg/day;and Combination group (n=32),patients were treated with benazepril 10 mg/day and candesartan 4 mg/day.The medication lasted for 8 weeks.Blood pressure(BP),plasma urinary albumin and creatinine ratio(A/C),endothelin-1(ET-1),nitrogen monoxide(NO)and potassium were examined and compared before and after the medication respectively. Results:The levels of BP,plasma A/C and ET-1 were decrease and NO was increased in all three groups(P<0.05,respectively );those changes were more obvious in Combination group.While plasma creatinine and potassium had not changed before and after the treatment(P>0.05,respectively). Conclusion:Effect of benazepril and candesartan combination in patients with hypertension-combined microalbuminuria was better than single medication in either benazepril or candesartan.
