【摘要】 目的:研究外周血核转录因子过氧化物酶体增殖物激活受体γ(PPARγ)mRNA表达与血清脂连素浓度和冠状动脉(冠脉)病变严重程度的相关性。方法:经冠脉造影检查后将142例患者分为冠心病组(经造影确诊冠心病者107例)和对照组(经造影排除冠心病者35例)。逆转录多聚合酶链反应法测定外周血细胞中PPARγmRNA表达,酶联免疫吸附剂(ELISA)法检测血清中脂连素含量;并根据造影结果对冠脉病变进行Gensini评分,分析PPARγmRNA、脂连素水平与冠脉病变Gensini评分的相关性。结果:冠心病组PPARγ和脂连素水平较对照组明显降低,差异具统计学意义(P均<0.05)。PPARγmRNA及脂连素与冠脉病变Gensini评分呈负相关关系(r=-0.898,r=-0.923,P均<0.05),PPARγmRNA表达和脂连素水平呈明显正相关关系(r=0.875,P<0.05)。结论:PPARγ和脂连素是冠脉粥样硬化的负调控因子,对冠心病高危人群具有保护作用。

【Abstract】 Objective: To investigate the relationship between the severity of coronary artery lesions to mRNA expression of peroxisome proliferator activated receptor γ(PPARγ) and the serum level of adiponectin in patients with coronary heart disease(CHD).Methods:According to the results of coronary angiography(CAG),142 subjects were divided into two groups.Normal control group(n=35);CHD group(n=107) that including 75 patients with acute coronary syndrome(ACS subgroup) and 32 patients with stable angina pectoris(SAP subgroup).The mRNA expression of PPAR γ in peripheral lymphocytes was evaluated by reverse transcription polymerase chain reaction(RT-PCR),the serum adiponectin was measured by enzyme linked immunosorent assay(ELISA).The severity of coronary lesions by CAG was evaluated by means of Gensini coronary score system.Results: Decreased mRNA expression of PPAR γ and serum adiponectin were observed in CHD group than in Normal control group(P<0.05).In addition,mRNA expression of PPAR γ and serum adiponectin in ACS subgroup were significantly lower than in SAP subgroup(P<0.05).Negative correlation was found between mRNA expression of PPAR γ,adiponectin and the Gensini score of coronary artery lesions(P<0.05).Meanwhile,positive correlation was found between mRNA expression of PPAR-γ and adiponectin concentration(P<0.05).Conclusion: PPAR γ and adiponectin were negative regulators for coronary artery lesions,it might be a valuable strategy to protect patients from CHD by up-regulating the expression of PPAR γ and adiponectin.
