胸腔镜下胸腔内热化疗对肺癌合并胸腔积液的治疗作用

【摘要】 目的探讨胸腔镜下胸腔内热灌注化疗对肺癌合并胸腔积液患者的治疗效果。方法40例肺癌合并胸腔积液患者于全身麻醉下用胸腔镜探察胸腔,并行胸膜活检,用人工心肺机将预充液(生理盐水3000mL+顺铂300mg)加热至42~44℃后,通过2根导管连接至胸腔内,循环灌注胸腔约60min。结果胸水控制有效率为97.5%,KPS评分升高率为67.5%;未见明显的毒副作用。结论胸腔镜下胸腔内热灌注化疗具有快速诊断、创伤小、控制胸水效果好、副作用小的优点。

【Abstract】 【Objective】To assess the comprehensive effect of pleural perfusion thermo-chemotherapy(PPTC) for malignant pleural effusion(MPE) caused by lung carcinoma under VATS.【Methods】40 patients with MPE caused by lung carcinoma underwent pleural biopsy of the primary lesions and PPTC under VATS without thoracotomy.PPTC consisted of irrigating the pleural space with 43.0℃ saline(3000 mL) solution containing cisplatin(300 mg) using a devised circuitcircular intrapleural hyperthermic perfusion with 42.0℃~44.0℃ warmed normal saline for 60 minutes.The control rate of pleural effusion,Karnofsky performance status(KPS) grade,side-effects of the patients were calculated.【Results】Treatment group achieved control rate of 97.5%.Increase of Karnofsky performance status(KPS) grade was 67.5%.There were no significant side effects.【Conclusion】Pleural perfusion thermo-chemothera-py under VATS is a new,safe,less invasive and more effective approach for malignant pleural effusion(MPE) caused by lung carcinoma.
