532nm半导体激光联合Nd∶YAG激光周边虹膜切开术治疗原发性闭角型青光眼
Semiconductor Laser Combined with Nd ∶YAG Laser in Peripheral Iridectomy for Treatment of Primary Angle-closure Glaucoma
【摘要】 目的探讨532 nm半导体激光联合Nd∶YAG激光行周边虹膜切开术治疗原发性闭角型青光眼的临床疗效。方法原发性闭角型青光眼患者68例76只眼,34只眼行单纯Nd∶YAG激光周边虹膜切开术,42只眼行联合激光周边虹膜切开术。观察术中能量,术后眼压恢复情况,术中、术后并发症,前房及房角情况。结果术前、术后1 h、1 d、1和4周的平均眼压值,联合激光组分别是(16.07±3.32)、(24.56±3.01)、(18.36±2.01)、(16.12±2.13)和(15.91±2.38)mmHg;单纯激光组分别为(15.98±2.11)、(28.12±3.37)、(19.52±2.47)、(17.22±2.24)和(15.87±3.41)mmHg,单纯激光组术后1 h、1 d的眼压升高幅度较联合激光组高。联合激光组周边虹膜均一次透切成功,前房出血较少,前房炎症反应较轻,无视力损伤及角膜、晶状体损伤。结论532 nm半导体激光联合Nd∶YAG激光行周边虹膜切开术具有并发症少、一次透切率高和总能量低等优势,是治疗原发性闭角型青光眼的理想术式。
【Abstract】 Objective To assess the efficiency of semiconductor laser combined with Nd ∶YAG laser for laser peripheral iridectomy(LPI).Methods Seventy-six eyes of 68 patients with indications were collected.Thirty-four eyes were performed peripheral iridectomy with semiconductor laser combined Nd ∶YAG laser.Laser power was recorded,complications were observed before and after operations.Their treatment results were compared with those who underwent Nd ∶YAG laser singly.Results The mean preoperative and 1 hour,1 day,7 day,1 month postoperative intraocular pressure(IOP) of the combined laser group was(16.07±3.32),(24.56±3.01),(18.36±2.01),(16.12±2.13)and(15.91±2.38) mmHg,while those of single laser group was(15.98±2.11),(28.12±3.37),(19.52±2.47),(17.22±2.24)and(15.87±3.41)mmHg.The mean 1 hour and 1 day postoperative IOP of the combined laser group was higher than that of single laser group.Peripheral iridectomy was performed successfully by once in these 34 eyes,fewer hyphema occurred,uveitis in anterior chamber was mild.And no visual disturbance and no cornea or lens injuries were observed in combined laser group.Conclusions The semiconductor laser combined with Nd ∶YAG laser in perpheral iridectomy is a safe and effective treatment with less complications,higher rate of once-perforation and lower laser power.It is good for primary angle-closure glaucoma.
